To Whom It May Concern:
On July 27, 2009 agents of l-odge Environmental Inc. entered the property located on 325 I
~oiJt.h 4-2nd Place Springfield, Oregon 97478 and conducted an asbestos abatement project The
project removed all asbestos materials called out by Lodge Environmental asbestos survey report.
The project was completed following Oregon DEQ, Oregon OSHA and LRAPA regulations. The
project was completed on July 28, 2009. Notification was filed with LRAPA and fee was paid. This
residence is free of asbestos and may now be demolished. If you have any further questions please
feel free to call on me.
