At the request of the tenant, Diane 8owers, the Springfield Building Safety Division
, conducted a Courtesy Inspection at the above referenced address.. The. following items
are deemed. hazardous and do not meet the minimum standards of the Springfield Housing
Code: ..<.:.

STRUCTURAL AND FIRE AND LIFE SAFETY
..
1. . The roof is leaking in numerous locations in. the attic'and dripping through the
. ceilings in the living room and bedroom. Repair or replace the roof that has
deteriorated.

2. The rear door located in the kitchen has been nailed in the closed position.
. Provide new hardware and move the refrigerator that is blocking use of this
door.

3. The concrete block chimney located in the living room does not have a liner and
shall not be used for the wood stove.. Disconnect the tenants wood stove and ce-
ment .the chimney opening closed.
.4. Prgvide facilities with a heating system capable of maintaining a temperature of
70 F. at a point 3 feet above the floor in all habitable rooms.

PLUMBING SAFETY
1. The kitchen. sink faucet does not work. Repair or replace this unit.to operate
properly.
2. The toilet seat has been broken off at the hinges and will require replacement.
ELECTRICAL SAFETY
1. Replace the defective switch and plug outlets in the bedroom and living room.
This is a potential electrical shock hazard.
2. Repair service equipment, close open holes and clamp cables properly at service
box.

3. Electrical boxes on paneled walls require extension rings to flush face of box
