Performed inspection on the reinforcing steel in concrete paving repair area south of
Umpqua Bank, Reinforcing steel was checked for size, grade, laps, quantities, locations,

Observed the. placement of 21,25 yards of Eugene Sand and Gravel 7sk, conci~te (mix.
396850) into area noted above, Concrete was placed directly from truck and was
mechanically consolidated, Cast one (1) set of five (5) 4" x 8" compressive :strength
cylinders, Notified Adam Hughes (Brown Contracting) of the on-site results, Slu'mp - 3";
concrete temperature -77F;. air temperature - 62F; airconteht - 4,6%, I',
Mr, Hughes was asked to notify our office when additional services are needed.
To the best of our knowledge, the work inspected was in accordance with plans dated
03-12-09 and applicable workmanship provisions of the I.B,C" except if noted ab'bve,
