Application is hereby made by the undersigned property for permi~sion
to connect to the following described pro~erty to a trunk line sewer 6wned
and maintained by the City of Springfield, and I agree to pay such a hookup
~harge of $3.40 per front foot of the pr06erty to be served by such trunk
line sewer in lieu fo an assessment against the described property.

Range 2 West of the WI I lamotte Meridian, thence South 89044' East 63.11
feet along the s~.Ith line of said hlghwey, thence South 150.0 feet, thence
West 63.11 feet, thence North 150.0 feet to the point of beginning in Spring-
