Donna Wallace
90660 Marcola Rd,
Springfield, OR 97478-8704
Dear Donna,
I'm writing regarding the Temporary-Use Emergency Medical Hardship permit that was issued
on May 23, 2008 that allowed a Sth -wheel trailer to be set up behind the residence at 1222 E
Street to assist Harry Wallace, The permit was issued for one year and that permit has expired,
The City Development Code allows for a six-month extension on the condition that the
attending physician for Mr, Wallace provides the City with a letter affirming that he continues
to require the use of the Sth-wheel trailer under the Emergency Medical Hardship permit,
I understand that Harry has passed away and that Monty Wallace continues to live in the S'h_
wheel trailer, working on repairs to the home, I spoke to Monty about his own medical needs
and the surgery that was planned for his cancer. Based on the need to repair the home and
Monty's health status, the Emergency Medical Hardship permit is extended until December
31,2009. Since Harry Wallace was the subject for which the original permit was approved, and
he has passed away, the permit cannot be extended beyond this date.
Please feel free to contact me if you have questions about this letter extending the permit,
