CITY OF SPRINGFIELD
APPLICATION AND AGREEMENT FOR SEWER HOOKUP
Application is hereby made by the undersigned property owner for permission
to connect the following described property to a city sanitary sewer line, owned,
and maintained by the City of Springfield, and I agree to pay such a hookup charge
of $4.50 per front foot of the property for the first 150 feet in depth to be
served by such city sewer line in lieu of an assessment against the described
property. An additional $0.03 per square foot will be charged for any additional
property beyond the first 150 feet.

Address: 5111 Main Street
Beginning at a point on the South line fo the McKenzie Highway No. 28, 534.25 feet
West of. the East line of the A. W. Hammitt Donation Land Claim No. 38, Township 17
South, Range 2 West of the Willamette Meridian, thence South 200 feet; thence West
70.0 feet' thence North 200 feet to the South line of said highway, and thence East
along the South line of said highway 70.0 feet to the point of beginning in Lane
County, Oregon

square feet @ $0.03 per sq. ft.
= $420.00
This agreement has been computed as being one-half (l,) of the equivalent cost'
of an eight (8) inch lateral sanitary sewer at the rate of $4.50 per abutting
front foot and docs not include the cost of a house connection to said city sewer,
sewer user charges, plumbing pennits or other such costs to be assumed by the
property owner.
