I have reviewed the materials provided with the subject appliCation for Public)Works land use
conditions. All conditions have been met with the exception to condition #5 below:
1. Condition: Prior to final plat, the applicant shall revise plans for "Y" c,onnection to occur
outside of the right-way (within thePUE) and obtain City approval fof:new location.
Response: Sewer connection has been relocated imd approved under City permit.
COM2009-00325. This condition has been satisfied. .
2. Condition: Prior to final plat, applicant shall obtain necessary permits including a Public
Works encroachment permit and successfully construct the proposed sanitary "Y"
connection per City approved plans.
Response: City permits have been obtainedfor sewer connection. This condition has
been satisfied.
3. Condition: Prior to final plat, the applicant shall obtain approval and permits from Lane
County Sanitarian for the decommissioning of the existing on site private septic system.
Response: A copy of the Lane County Sanitarian Septic System decohtmissioning form
with acknowledgement was submitted. This condition has been satisfied.
4. Condition: Prior to [mal plat approval, the applicant shall submit an improvement
agreement for extending the sanitary sewer along Del Rose A venue to the western
boundary of the developed property as required by the City of Springfi~ld Engineering
Design Standards and Procedures Manual Section 2.02.1.
Response: A copy of the signed improvement agreement was submitted with the plat
application. This condition has been satisfied
5. Condition: Prior to final plat, a private utility (sanitary) easement will be necessary and
recorded with the plat for portions of the proposed 6" sanitary pipe that will encroach
onto adjacent parcel I for the benefit of parcel 2.

shown located within the Public Utility Easement (PUE) which is not allowed as public

and private easements shall be separated. Final plat plan will need to be corrected.
