thatthe above building has been approved by Facilities PlanninlJ and Safety.

NOTE: Assisted Livinl{ anc! Residential Care FaCilities require licensure by Seniors and
People with DIsabilities (SPD) prior to occupancy by residents. New Ambulatory
Surgery Centers and new Hospitals require licensure by Licensing and Certification,
Oregon Health Services (OHS) prior to occupancy by patients.
