Performed continuous inspection during the installation of eight (8) 1/2" x 7" Simpson
wedge anchors into the concrete slao for the wood stair to slab connection detail 1/S-
3.1 at stair W123 and, W124, four (4) at each location. Prior to installation, the
predrilled holes were cleaned with compressed air. Verified minimum emoedment of 2
3/4".
Bill (AJT Construction) was asked to notify our office when additional services are
needed.
To the oest of our knowledge, the work inspected was in accordance with drawings
dated 03-25-09, engineer memo dated 04-21-09 and applicaole workmanship
provisions of the I.B.C., except if noted aoove.
