a.m. will be made the same working day, inspections requested after 7:00 a.m. will be made the following

Sign Final: After all required inspections are conducted and approved and the sign installation is completed.
By signature, I state and agree, that I have carefully examined the completed application and do hereby certify that all
information hereon is true and correct, and I further certify that any and all work performed shall be done in accordance with
the Ordinances of the City of Springfield and the Laws of the State of Oregon pertaining to the work described herein, and
that NO OCCUPANCY will be made of any structure without permission of the Community Services Division, Building Safety.
I further certify that only contractors and employees who are in compliance with ORS 701.005 will be used on this project.
I further agree to ensure that all required inspections are requested at the proper time, that each address is readable from the
street, that the permit card is located at the front of the property, and the approved set of plans will remain on the site at all
times during construction.

This Sub-Lease Agreement is made and entered into on 151 day of October 2007
between The Arc of Lane County, hereinafter referred to as "Lessor" and The Arc of
Benton County, "Bingo at the Bank" hereinafter referred to as "Sub-Lessee."
Sub-Lease Agreement
WITNESSETH: The Lessor sublets to the Sub-Lessee the following described property
as the Bingo Hall located at 4181 EStreet, Springfield, Oregon 97478.
1. TERM: This Sub-Lease shall run for a period of seven years, commencing on the
first day of October 2007 and expiring on the 30th day of September 2014.
2. PURPOSE: The premises shall be used by The Arc of Benton to conduct Charitable'
Gaming and associated fund raising activity at least three evenings per week.
3. RENT: Sub-Lessee shall pay to Lessor the sum of $3,500 for year 1 and 2. The rent
shall be increased by 3% per year, year 3 - 7. Rent shall be due and payable on the
last day of the month, in advance, to the following address designated by the Sub-
Lessor:4181 E Street, Springfield, Oregon. .
4. PROPERTY TAX: Sublessor shall pay lessor 50% of assessed property tax on the
bingo portion of the building.
5. UTILITY COSTS: Sublessor will pay Lessor 50% of the 33% of the total utilities bill
for the building.
6. PROPERTY INSURANCE: The Lessor shall maintain property insurance in the
amount required by the Owner. In addition, Lessor shall maintain insurance on
contents of the Bingo Hall sufficient to replace concessions and gaming equipment.
. Sublessor will pay Lessor 50% of said cost associated only with bingo facility.
7. GENERAL LIABILITY INSURANCE: Each party to this agreement shall maintain a
general liability policy in the amount of $2,000,000 general aggregate and Worker's
Compensation. . .
8. CERTIFICATE OF INSURANCE: Parties shall provide each other with Certificates
of Insurance, noting the other party as "other insured." . .
