Periodic Inspection: 0
Performed inspection on the reinforcing steel for slab on grade (4") at office area and
slab on grade (6") for carwash area. Reinforcing steel was checked for grade, size,
lap, placement and clearance. Reinforcing steel was secured and supported above
grade on concrete dobie blocks. I
Observed the placement of 30.0 of 38.0 yards of Knife River 3500 psi concrete (mix 35-
IN) in area noted above. Concrete was placed by pump and was manually
consolidated. Cast one (1) set of four (4) 4" x 8" concrete test cylinders from load 1.
Slump - 5"; air temperature - 47F; concrete temperature - 67F
Ron (Mcintyre Construction) was notified of our on-site test results.

To the best of our knowledge, the work inspected was in accordance with plans dated
04-10-08 and applicable workmanship prov,isions of the I.B.C., except if noted above...

Inspection of: Masonry Weather: Overcast . Continuous Inspection: 0
Contractor: WildishBuilding Co. BPNo.: 2008-00225 Periodic Inspection: 0
Performed inspection on the reinforcing steel in 6" CMU wall in the coagulant building
(8' to 12'). Reinforcing steel was checked for grade, size, lap, placement and
clearance, where applicable. Reinforced cells were relatively free of excess mortar
projections.
Observed the placement of site batched high strength Sakrete Concrete Mix by bucket
and mechanically consolidated after placement. Only reinforced cells and bond beams
were grouted solid. Slump - :t9"; air temperature - 52F; grout temperature - 54F.
Jessie (Art's Masonry) was asked to notify our office when additional services are
needed.
To the best of our knowledge, the work insbected was in accordance.with plans dated
02/2008, masonry submittals and applicaple workmanship provisions of the I.B.C.,
except if noted above. ,I

Performed preliminary reinforcing steel inspection for 6" CMU wall in coagulant building
(slab to 4'). Reiriforcingsteel was checked for grade, size, lap, placement and
clearance, where applicable. Reinforced cells were relatively clean and free of excess'
mortar projections. The un-reinforced cells were blocked off with grout stop. Final
inspection will be performed prior to grout placement.
To the best of our knowledge, the work inspected was in accordance with plans dated
02/2008 and applicable workmanship provisions of the I.B.C., except if noted above.
