No.; Type of Inspection: Z4175LD Epoxy Building Permit No.: COM2008-01595 Weather: Inside Field Observations and Comments Performed continuous inspection during the installation of #4 epoxied rebar dowels in the following locations: section at HSS post bases (SK-2) 6 total (3 on each side of door opening). Prior to installation the predrilled holes were cleaned using a nylon brush and compressed air. Verified minimum embedment depth of 6 inches. The above listed epoxy anchors were installed using Hilti Hit 150 epoxy. The epoxy anchors were installed in accordance with latest revisions dated 1/31/09 from Metzler Engineering. To the best of our knowledge, the work inspected was in accordance with the building department approved design drawings, specifications and applicable workmanship provisions of the International Building Code. Contractor was asked to notify our office when additional services are needed. Reinforcing steel was checked for grade, size, lap, placement and clearances where applicable. Reinforcing steel was secured using tie wire and supported on concrete dobie blocks. Reinforcing steel was placed in accordance with approved design drawings, specifications and engineering approval for as-built. Performed visual inspection durinG placement of concrete in the locations listed above. Concrete was pi aced by pump and mechanically consolidated. Cast one (1) set of four (4) (4x8) concrete test cylinders. Contractor was asked to notify our office when additional services are needed.
