Observed the installation of fifty-two (52) 5/8" x 7" to 9" epoxy all thread anchors into
CMU wall near roof line for L TT20B Simpson Straps along the perimeter walls of
building. Prior to installation, the predrilled holes were cleaned with a brush and
compressed air. Verified minimum embed depth of 6". Anchors were secured with
Simpson Set 22 epoxy adhesive (ICC ES 1772/ expiration date 05-31-09).
Bob (Mcintyre Construction) was notified of on-site observations and was asked to
notify our office when additional services are needed.
To the best of our knowledge, the work inspected was in accordance with plans dated
04-10-08 and applicable workmanship provisions of the I.B.C., except if noted above.
