Application Fee - refer to the Development Code Fee Schedule .for the appropriate fee
calculation formula. A copy of the fee schedule is available at the Deyelopment Services
Department. Any applicable application, technology, and postage fees, are collected at
the pre-submittal and submittal stages.
Land Division Plat Application Form ..
Letter Addressing Conditions of Approval - lists and addresses each condition of
approyal, detailing the actions taken and current status of each item.
. Nine (9) Paper Copies of the Plat stamped and signed by the surveyor.
Two (2) Copies of Closure Sheets for the boundary and each lot or parcel and all
common areas, dedicated areas, and easement areas that are.not simple parallel offsets.
Two (2) Copies of Title Report or Subdivision Guarantee for the parcel being .
diyided. The title report must be dated within 30 days of submittal at the time of the final
submittal. An older report is OK at the pre-submittal stage.

Two (2) Copies of Each of the Reference Documents and Plats listed on the plat;
Two (2) Copies of Each of the Supporting Documents - the vesting deed (must vest
title to the owner listed on the plat), existing easement deeds, and documents listed as
exceptions in the title report, etc.

Two (2) Draft Copies of Any New Easements or Restrictions being' created by
separate document, improvement agreements, maintenance agreements, joint use

FOR THE FINAL SUBDIVISION PLAT Al'PLicA nON
This application is a submittal of the final subdivision plat for the property located south
of Mt.Vemon Road and west of the future Jasper Road Extension. The subdivision was granted
conditional approval on February 17, 2006 under Case Number SUB2006-00004.
The following is a discussion of how the final plat conforms to the conditions of approval
placed on the tentative subdivision application: .

The segment. of the public street extending south from South 57th Place, and located
between Cinder Street and South 58th Street, shall be named Obsidian Avenue on the Phase
6 Final Plat:
This condition was addressed on Jasper Meadows Fifth and Sixth Additions asfollows: The
requested street name is reflected on the plat.
2) The proposed subdivision plan shall be revised to indicate street tree locations within the
frontage of Lots 307 and 308, the frontage of Tract' A', and along the south side of Orchid
Way.
Lots 307 and 308 have been renumbered as Lots 308 and 309. Sheet CI8 of the Public
Improvement Plan, PIP, shows street trees to be planted within the frontage of Tract 'A'
and along the south side of Orchid Way during construction of the public improvements.
Sheet CI8 shows future street tree locations for all other lots, including renumbered Lots
308 and 309.
3) Prior to approval onhe Final Plat for Phase 7, street trees shall be installed within the
frontage of Tract' A' concurrent with installation of curb, gutter, curb cuts and sidewalks.
This condition was addressed on Jasper Meadows Fifth and Sixth Additions .as follows: As
discussed in the previous condition, the PIP shows these street trees to be planted during
construction. These improvements have been bonded by the developer to allow recordation
of the plat prior to completion of the improvements.

. prepare and submit a comprehensive street tree plan describing the location and species of.
proposed street trees. This plan will be used as the basis Jor a street tree performance bond
as residential development proceeds in the subdivision phases over the next several years.

Prior to approval of the Final .Plat for Phase. 8, the developer shall install the paved
pedestrian accessway (shown in the detail on Sheet 3) within the easement between Lots
293 and 294, and between Lots 285 and 286. '
Lots 293 and 294 have been renumbered as Lots 294 and295;' Lots 285 and 286 have been
renumbered as Lots 286 and 287, The paved pedestrian wily has been or will be installed.
These improvements have been bonded by. the developer to allow recordation of the plat
prior to completion of the improvements.
The grading interface between the sidewalk along the west side of South 57th Street and the
future public park area shall be revised to match the elevation of the park landscaping
surface with the sidewalk. The grading adjustment shall not create more than a 1 % cross-
fall grade across the park surface to the wetland area along the western edge. Details of the
park and adjacent sidewalk grading shall be provided on the final engineering plans for
Phase 6.

This gr:ading is shown on Sheet EC2 of the PIP. Willamalane Park & Recreation District
provided written concurrence for the PIP prior to construction. These improvements have
been bonded by the developer to allow recordation of the plat prior to completion of the

Prior to approval of the Final Plat, the developer shall install a 10- foot wide paved
pedestrian accessway connecting Puinice Place and South 57th .Place within the public
utility easement between Lots 230 and 231, and between Lot 211 and the stormwater
outfall. The pedestrian accessway shall be combined with the proposed IS-foot stormwater
outfall access driveway for the portion along the north boundary of Lot 211. The entire
length of the combined pedestrian and stormwater access driveway shall be paved.
This condition was addressed on Jasper Meadows Fifth and Sixth Additions as follows:
Lots 230.and 231 have been-renumbered as Lots 231 and 232; Lot number 211 has been
renumbered as Lot 212. Sheet C14 of the PIP. shows a 10.00' paved walkway within a
15.00' right-of-way and a 14.00' easement on renumbered Lots 231 and 232. Sheet C16 of
the PIP shows cross-sections for both segments. of the walkway. The northern segment that
is combined with the access drive has a 15.00' wide all_weather driving surfac~ (10.00' of
concrete with 2-1/2' of gravel on each side). The southern segment has only 10.00' of
concrete. These improvements have been bonded by the developer to allow recordation of
the plat prior to completion of the improvements.

The developer shall provide a detail on the proposed 48-inch stormwater pipe, including
.the rip-rap outfall, pedestrian accessway crossing, adjacent grading, and appropriate
lockable trash rack/sewer grate cover, with the final engineering submittal.
This condition was addressed on Jasper Meadows Fifth and.Sixth Additions as follows:
Sheet C15 of the PIP has a plan/profile view of the pipe, outfall, pedestrian crossing, and
adjacent grade. Sheet C16 of the PIP has details of the pipe trench and pipe outlet.
Portions of Cinder Street, Pumice Place, Mica Street, South 58th Street and South 57th Place
that have 28-foot pavement widths shall have appropriate "No Parking - Fire Lane"
signage posted on one side of the street in accordance with SUFC 902.2.2.1.

The required signs are shown on Sheet C18 of the PIP. These improvements have b~en
bonded by the developer to allow recordation of the plat prior to completion of the
improvements.

The pedestrian accessway and stormwater outfall driveway access between Pumice Place
and South 57th Place shall be installed within a separate, dedicated public right-of-way and
not part of the tract dedicated to Willamalane Park & Recreation District.
This condition was addressed on Jasper Meadows Fifth and Sixth Additions as follows: The
plat offers a 15.00' right-of-way dedication between Tract 'A' of the 5th Addition and
renumbered Lot 212 (formerly numbered Lot 2J1). .

Prior to approval of the Final Plat for Phase 6,. the developer shall provide copies of the
proposed drainage and access easement documents affecting the north park and wetland
area.
This condition was addressed on Jasper Meadows Fifth and Sixth Additions as follows: The
drainage easements are dedicated to the public on the plat and referenced in the text of the
Bargain and Sale Deed that is noted in Condition No.9. The access easement is dedicated
as public right-of-way on the plat as called in Condition No.1 5 above.
17) Th.e developer shall provide a stub for water, electrical and sanitary sewer to the back of
sidewalk within the north public park that is to be dedicated to Willamalane Park &
Recreation District.
This condition was addressed on Jasper Meadows Fifth and Sixth Additions as follows:
Sheet C2 of the PIP shows a sanitary sewer stub extended to the park The construction
inspector for the project has stated that the electric and water stubs have been provided as
well. These improvements have been bonded by the developer to allow recordation of the
plat prior to completion of the improvements.
18) Prior to approval of the Final Plat, a Geotechnical Report shall. be provided for all home
building sites identifYing the specific soils with a recommendation for the foundation. Fill
placed on residential lots shall be documented in the building pad area. The Geotechnical
Report shall clarifY which residential lots require further geotechnical investigation and

19) Access to subdivision lots.in Jasper Meadows Phases 6, 7, and 8 shall be restricted to one
driveway per lot except where duplex units are permitted and duplexes are built, or as
otherwise granted by the Public Works Department under permit.
This condition is addressed by a note on the plat restricting all lots to single driveways
except duplex lots where duplex ,units are built which shall be allowed two driveways, 'or as
otherwise granted by the Public Works Department under permit.
