Field Observations and Comments .
Performed continuous inspection during the installation of five (5) 5/8" and two (2) 3/4" diameter all thread epoxied
anchor bolts in the following locations: Existing east CMU wall on line 9 between C and D at door header. Prior to
installation the predrilled holes were cleaned using a nylon brush and compressed air. Verified minimum embedment
depth of 5 inches for the 5/8" and 6 3/4" for the 3/4". The above listed epoxy anchors were installed using Hilti Hit HY
150. Contractor stated the nuts would be tightened after epoxy has cured. To the best of our knowledge, the work
inspected was in accordance with the building department approved design drawings detail C/S2, specifications and
applicable workmanship provisions of the International Building Code.
