Dear Mr. Hawes,
This letter confirms that FEI Testing & Inspection, Inc: performeb special inspection ;ervices in accordance
with IBC and as directed by our client for the above-referenced project. We were employed to provide
continuous or periodic inspection on specific dates, as documented per our special inspection reports for:
Oearthwork
0structural masonry
0epoxyanchors/dowels
~reinforced concrete
~bolts installed
in concrete
~structural welding (shop/field)
~high-strength bolts
Ospray applied fireproofing
~ ceiling grid

Based upon our observations and written reports of this work, it is our judgment that the inspected work was
performed, to the best of our knowledge, in accordance with the a'pproved plans, specifications, change
orders and/or structural en'gineer's instructions, and applicable provisions of Chapter 17 of.the Internationai
Building Code, with the following exception: revised roof deck t~lwall attachment per RFI195 is pending in
future tenant space 380. . ;,
Inspection on the revised roof deck to wall attachment per RFI195 has been completed. To the best
of our knowledge, there are no other outstanding "special inspection" issues on this project.

Inspection of: Epoxy Anchorsl Welding Weather: Overcast Continuous Inspection: 0
Contractor: Chambers Construction BP No.: COM2007-01851 Periodic Inspection: ~
Performed continu~us inspection during the installati~n of six (6) .1/2" x 7" all thread
anchors into the concrete wall at grid lines 5.1 roof level. Prior to installation the
predrilled holes were cleaned with a brush and compressed air. Verified minimum
embedment of 6". Anchors were secured with Epcon A7 epoxy (ICC ESR.2351/
expiration date 03-25-09).'
Also, performed visual inspection on the angle to deck:welds per RFI #195. Completed
welds were checked for size, length, profile and locatiqn.
. Contractor was asked to notify our office when additional services are needed.

To the best of our knowledge, the work inspected was in accordance with RFI #195
dated 11-20-08,- applicable AWS D1.1 and applicable workmanship provisions of the
IBC, except if noted above.
