Observed the placement of 29.0 yards of Knife River 3000 psi concrete (mix 30-1V) for
perimeter continuous footing and one (1) interior continuous footing, the north wall of.
car wash area. Concrete was placed by pump and mechanically consolidated. Cast
one (1) set of four (4) 4" x 8" concrete test cylinders. The on-site test results from load
1are as follows: concrete temperature- 51F; air temperature- 36F; air content- 5.5%;
slump- 4". The reinforcing steel for areas noted above was checked by our office on
02-09-09 (reference FEI report E-22357).
Bob (Mcintyre Construction) was advised of the on-site test results and was asked to
notify our office when additional services are needed. .
To the best of our knowledge, the work inspected was in accordance with plans dated
04-10-08 and applicable workmanship provisions'.of the I.B.C., except if noted above.

Performed inspection on reinforcing steel for perimeter continuous footing and one (1)
interior continuous footing at north wall of car wash area. Reinforcing steel was
checked for grade, size, lap, placement and clearance. The #4 dowels for tie between
. slab and footing are not installed. Contractor stated doweis will be epoxied at a later
date and would contact EOR regarding embed depth of dowels.
Mcintyre representative was asked to notify our office when additional services are
needed.
To the best. of our knowiedge, the work inspected was in accordance with plans dated
. 04-10-08 and applicable workrpanship provisions of the I.B.C., except if noted above.
