inspections required by the City of Springfield -Community Services Division.

4. Structural Masonry
5. Welding
6. Epoxy Anchors
Based upon PSI's field reports of this work, it is my judgement that the
inspected work was performed, to the best of my knowledge, in accordance with the
approved plans, specifications and the applicable workmanship provisions of the
Uniform Building Code.
