To comply ,vith the Special Inspection and Testing as required iri the permit application, we have
made the following arrangements:
I, Professional Service Industries, Inc.
A purchase order was issued to PSI for:
Determination of the strength of the bolts used in the roof repair.
Welding inspections using non destructive testing,
Concrete strengths detennination for the concrete slab the new scrubber will sit on,
At this time, the determination of the strength of the bolts used in the roof repair has been
completed,
About one-half of the welding inspection has been completed,
The concrete slab for the scrubber has been poured and the cylinders have been submitted
to PSI for strength detennination,
We are expecting their completed report in about two weeks and we will forward a copy
to you ASAP,
2, Evergreen Engineering, Inc.(EEI)
EEl will:
Oversee the driving ofthe piles for the scrubber slab.
Inspect th~ weldment plates to the anchors. Evaluation of the welds and the
adherence to drawing # 1 OOCO I.

LARSON OF EVERGREEN ENGINEERING, ALBERT ALBUSH OF SPECIALTY CONTRACTORS,
AND BOB. BARNHART OF CITY OF SPRINGFIELD, DISCUSSED SPECIAL INSPECTION
REQUIREMENTS FOR HIGH STRENGTH BOLTING AND WELDING. BOB BARNHART STATED
THAT CITY REQUIRES ULTRASONIC TESTING ON ALL FULL-PENETRATION WELDING.
THE FOLLOWING SCOPE WAS DEFINED:
:1:, PSI ANTICIPATES 2 TRIPS TO THE SITE TO PEFORM ULTRASONIC TESTING ON
:1:00% OF FULL-PENETRATION WELDS.
2. PSI ANTICIPATES :1: TRIP TO PERFORM VISUAL INSPECITON ON :1:00% OF FILLET
WELDS,
3, PSI ANTICIPATES :1: TRIP TO THE SITE TO CHECK THE LEJUNE BOLTS FOR
REQUIRED TENSION WITH A SKIDMORE TENSIONING INDICATOR DEVICE AND TO
VISUALLY INSPECT :1:00% OF HIGH STRENGTH BOLTS INSTALLED.
PSI OBSERVED THAT SOME TO THE FULL-PENETRATION WELDS PERFORMED TO DATE ARE
NOT IN ACCORDANCE WITH DRAWINGS PROVIDED, PSI REP~ESENTATIVE REQUESTED
THAT SPECIALTY FAE OBTAIN PAPERWORK FROM THE ENGINEER OF RECORD ACCEPTING
PLAN CHANGES.
PSI REPRESENTATIVE ALSO REQUESTED THAT SPECIALTY PREPARE A SUBMITTAL OF
WELDER QUALIFICATIONS AND WELD PROCEDURERS,
