Performed visual inspection on the rebar placement in' the continous footing of the office
addition on the southwest corner of existing structure. The rebar was secured with tie wire

and supported with concrete blocks and placed in accordance with plans and detail 4/5 dated
February 05, 1996 drawn by'Architect Jody Heady. Contractor was asked to notify our
offjce when additional services are needed.
