Epoxy Anchored Bolts

Report.
Witnessed the installation of fourteen (14) epoxy anchored bolts in the column footings at
the north, south and west entrances and at the southeast building corner. The' 10" x 3/4" all
thread bolts were embedded a minimum of 5" in pre-drilled 7/8" holes and were installed .
with Epcon System Ceramic 6 formula epoxy. . All holes were cleaned with a plastic brush
and compressed air prior to i.nstalling bolts in accordance with manufacturer's directions and
in accordance with the approved plans and specifications. Contractor stated bolts will be
tightened after adhesive .has set. Contractor asked to notify our office when additional JII

Report
Witnessed the installation of the epoxy anchor bolts for the interior and drive through
column base plates. The 10" x 3/4" all thread holts were embedded a minimum of 5" in
, pre-drilled 7/S" holes and were installed with Epcon System Ceramic 6 formula epoxy. All
holes were cleaned with a plastic brush and compressed air prior to installing bolts in
accordance with manufacturer's directions and in accord.a..nce with the approved plans an:J
specitications. Contractor stated bolts will be tightened after adhesive has set. Contractor
