This letter confirms that FEI Testing & Inspection, Inc. performed special inspection services
in accordance with IBC and as directed by our client for the above-referenced project We
were employed to provide continuous or periodic inspection on specific dates, as documented
per our special inspection reports for:
Oearthwork
Ostructural masonry
0epoxy dowels/anchors
. Oreinforced concrete
Obolts installed
in concrete
Ostructural welding (shop/field)
Ohigh-strength bolts
Dspray applied fireproofing
Based upon our observations and written reports of this work, it is our judgment that the
inspected work was performed, to the best of our knowledge, in accordance with the approved
plans, specifications, change orders and/or structural engineer's instructions dated July 02,
2007, and applicable provisions of Chapter 17 of the International Building Code, unless
otherwise noted or referenced above,

Substitutions and deviations from approved drawings
Dear Plans Examiner:
The Contracter has requested a substitution for the specified "Simpsen"epoxy adhesive fer the
anchors at the everhead doer jamb framing. The Centracter may use "Unitex Pre-Pexy" 300 Fast
#0162483, ASTMC881-90, Type 4, et the Centracter may use any "Hilti"er "Ram set"
epoxy or adhesive for the installatien of the anchers at the deer jamb framing.
The embed depth for the anchors at the jamb framing is sufficient at 3 5".

I notice there is no indication on the city status report that you received a report from
FEI for epoxy dowels/anchors, Therefore, I am enclosing copies of two letters that I
received: Branch Engineering for epoxy substitution and FEl's inspection report,
Please let me know if you need additional documentation, If not, I am assuming that
Branch's letter and FEl's report is adequate.
