Inspection of: Reinforcing Steel Weather: Showers Continuous Inspection: 0
Contractor: Hyland Construction BP No,: COM2007 -00369. Periodic Inspection: 0
Performed preliminary inspection on the reinforcing steel for perimeter continuous and
pad footings and interior pad footings. Reinforcing steel was checked for size, grade,
laps, quantities, locations. ties, clearances and supports. At the time of inspection
approximately 25% of the anchor bolts and hold-downs were in-place. A final
inspection will be performed prior to concrete placement.
Dave Shaw (Hyland) was asked to notify our office when additional services are
needed.
To the best of our knowledge, the work inspected was in accordance with plans dated
02-09-07, revision dated 04-18-07, addendum dated 05-08-07 and applicable
workmanship provisions of the I.B.C., except if noted above.

Performed final inspection on the reinforcing steel for perimeter continuous footings,
. five (5) interior column spread footings and two (2) exterior spread footings for canopy
columns. Reinforcing steel was checked for grade, size, lap, placement and clearance.
Reinforcing steel was secured with tie-wire and supported above grade.
Observed the placement of 30.0 cuJyds, of Knife River 3000 psi concrete (Mix 30-1N)
in areas listed above. Concrete was' placed by pump and was mechanically
consolidated, Cast one (1) set of four (4) 6" x 12" concr~te test cylinders, Dave Shaw
(Joh'n Hyland Construction) was notified of on-site test results: slump 4 1/2"; concrete
temperature 68F; air temperature 58F.
Mr. Shawwas asked to notify our office when additional services are needed,
