I Performed periodic visual inspection of the following items: Single pass seam welds and nelson studs at roof decking
area #2 from gridline E to F (+4') between 5 and 6 and single pass seam welds only at roof decking area #2 from
gridline E to H between 6 and 8 and F (+4') to H between. Weldments were inspected for size, length, profile, and
locations. To the best of our knowledge, the work inspected above was in accordance with the building department
approved design drawings, specifications and applicable workmanship provisions of the International Building Code and
applicable AWS D1.1 and AWS 01.3 codes. Contractor was asked to notify our office when additionai services are
needed.

Performed visual inspection of reinforcing steel and embeds at the 2nd floor at gridline E to M 5 to 8 stairway #1 gridline
F to G at 5.3 to 6, stair way #3 grjdiine B to 8.5 at 5.5 to 6 and slab on grade ramp at gridline G to H at 5 to 5.2.
Reinforcing steel was checked for grade, size, lap, piacement and clearance where applicable. Reinforcing steel was
secured using tie wire. Reinforcing steel was placed in accordance with iatest revisions dated 5/09/08. Additional
comments: The 2nd floor at gridline E to A at 5 to 8 reinforcing needs to be elevated to the required clearances
during placement of concrete. Observed at the 2nd floor the excess trash need to be removed prior to
. placement of concrete.
