No.: Weather: P0272-334-0809 Overcast Field Observations and Comments Performed continuous inspection during the installation of #4 (floors) and #5 (walls) epoxied rebar doweis in the following locations: Building #1 Kitchen and Music Rooms. Floor between kitchen and music room #4 rebar (19 east and 2 west). North wall of music room #5 rebar (2 center and 11 west). East wall of music room #5 rebar (14). East wall of kitchen #5 rebar (10 north and 8 south). Floor between kitchen and commons area #4 rebar (4 east and 2 west). South wall of commons area #5 rebar (30). Prior to installation the predrilled holes were cleaned using a nyion brush and compressed air. Verified minimum embedment depth of 5 inches. The above listed epoxy anchors were installed using Hilti Hit-Re 500-SD epoxy with ICC #2322' expiration: 9/2009. The epoxy anchors were installed in accordance approved job-site plans and manufacturer's directions. Contractor was asked to notify our office when additional services are needed. To the best of our knowledge, the work inspected above was in accordance with the building department approved design drawings, specifications and applicable workmanship of the International Building Codes. No.: Type of Structure: Type of Inspection: P0272-334-0809 Permit No.: Weather: COM2008-00437 Clear Reinforced Concrete Field Observations and Comments Performed visual inspection of reinforcing steel at the following locations: stage wall reinforcing at gridline E to E.l at 2 to 5 and curb reinforcing at gridline D tot D.3 at 2.5, D.5 to Eat 2.5, D to D.3 at 5.5 and D.5 to Eat 55 prior to piacement of concrete also placed concrete at column pour backs at gridline D.2 at 5, D.4 at 5 and D.6 at 5. Reinforcing steel was checked for grade, size, placement and clearances where appiicable. Reinforcing steel was secured using tie wire and supported on piastic chairs. Reinforcing steel was placed in accordance with the latest revisions dated 5/1/08. Performed visual inspection during placement of concrete in the iocations listed above. Concrete was placed by boom pump and mechanically consolidated. Cast one (1) set of our four (4) (4x8) concrete test cylinders. Additional Comments: Also placed concrete at the column pour backs at gridline C at 4, B at 6, B at 7, C at 6 and Cat 7. Contractor was asked to notify our office when additional services are needed. To the best of our knowledge, the work inspected above was in accordance with the building department approved design drawings, specifications and appiicable workmanship provisions of the International Building Codes.
