Inspection was made on a work in-progress welding panel cord splice connection in the administration wing entry and
library, between gridiines L to J - 3 to 6. Also inspection was made on weldments being placed at beam clips to beams
connection in the kitchen ciassroom wing from grid lines 2 to 5 - A to D. Contractor was asked to notify our office when
additional services are needed. To the best of our knowledge, the work inspected above was in accordance with the
building department approved design drawings, specifications and applicable workmanship provisions of the
International Buildin9. Codes.

Performed continuous visual inspection of muitipass fillet and fiare-bevel weldments for chord splices and panel
connections in gymnasium. Also, performed periodic visual inspection of Nelson studs, seam weids, and puddie welds
for pan decking above kitchen. Weldments were inspected for size, length, profiie and locations. Contractor was asked
to notify our office when additional services are needed. To the best of our knowledge, the work inspected above was in
accordance with the building department approved design drawings, specifications and applicable workmanship
provisions of the International Building Codes.

Performed continuous inspection during the installation of #4 epoxied rebar dowels in the following location: (46) East
wall of gym; (32) South wall of storage; (7) East wall of storage. Prior to installation the predrilled holes were cieaned
using a nylon brush and compressed air. Verified minimum embedment depth of 5 inches. The above listed epoxy
anchors were installed using Hilti Hit-RE 500-SD epoxy with ICC# ES-ESR-2322 (expiration 07/2009). The epoxy

anchors were installed in accordance with the approved job-site pians and manufacturer's directions. Contractor was
asked to notify our office when additional services are needed. To the best of our knowledge, the work inspected above
was in accordance with the building department approved design drawings, specifications and applicable workmanship

Verified minimum embedment depth of 5 inches. The above listed epoxy anchors were installed using Hit-RE 500-SD.
The epoxy anchors were installed in accordance with the approved job-site plans and manufacturer's directions.
Additional comments: welding continues to be monitored in the gym classroom wing on the panel cord splices and beam
clips from gridline 1 to 5 - E to G.5. Contractor was asked to notify our office when additional services are needed. To
the best of our knowledge, the work inspected above was in accordance with the building department approved design
drawings, specifications and applicable workmanship provisions of the International Building Codes.

2 to 5 - H to G.5, and along lines 5, 1, G.5 from 1 to 5 - E to G.5. Reinforcing steel was checked for grade, size, lap,
placement and clearances where applicable. Reinforcing steel was secured using tie wire and supported on concrete
dobie blocks. Reinforcing steel was piaced in accordance with approved design drawings and specifications. Additional
comments: welding of clips and panel cord splices is being monitored in 'the gym from gridlines 1 to 5 - E to G.5.
Contractor was asked to notify our office when additional services are needed. To the best of our knowledge, the work
inspected above was in accordance with the building department approved design drawings, specifications and
applicable workmanship provisions of the International Building Code.

strength Lajune bolts. Three (3) bolts were instailed in a Skidmore Wilhelm 'Tension indicator and tightened to twist-off.
Bolts tested met and/or exceeded the minimum kip value of 39 kips for 7/8 bolts. Contractor was asked to notify our

Performed visual Inspection of reinforcing steel at the following locations: concrete column piers per detail 1/106065-12
located at G.5 at 11,12,13, Eat 11, 12, 13, G to H at 5' 8" E of A line and C to D at 5' 8" E of A line. Reinforcing steel
was checked for grade, size, lap, placement and clearances where applicable. Reinforcing steei was secured using tie
wire and supported on concrete dobie blocks. Reinforcing steel was placed in accordance with approved design
drawings and specifications. Performed visual inspection during placement of concrete in the locations listed above.
Concrete was piaced by truck chute and consolidated. Additional comments: water migrated into hole during placement
of concrete, contractor made effort to displaced water with concrete. At completion of piacement contractor consolidated
concrete with vibrator. Contractor was asked to notify our office when additional services are needed. Tci the best of
our knowledge, the work. inspected above was in accordance with the building department approved design drawings,
specifications and applicabie workmanship provisions of the International Building Codes, except as noted.

As requested, a representative of PSi was on-site to observe boring for augered piers. WE observed augering of six (6)
piers on the west side of the structure for the covered play area. The following iocations were augered with a minimum
2 feet embedment into the well-graded bar run gravels. Depth of each hole is indicated in parentheses E at 13 (7' ), E at
12 (9 W), Eat 11 (7'), G.5 at 13 (8 W), G.5 at 12 (8') and G.5 at 11 (8'). The holes were drilled using a 2' auger. Far
(4) additional 12" wide piers were drilled for staircases on the northeast and southeast sides of the structure, 2 on each,
side. From north to south depths drilled to are as follows: 8 W, 8', 12' and 12'. All holes had standing water by end of
drilling. To the best of our knowledge, the work inspected above was in accordance with the building department
approved design drawings, specifications, geotechnical report and applicable workmanship provisions of the
International Buildinq Codes.

Performed visual inspection of reinforcing steel at the following locations: pour back strips gridline J to L at 3 to 5 and J
to K at 5 to 6 grade beams E at 11 to 13 and G.5 at 11 to 13 and roof at B to D at 2 to 5 prior to placement of concrete.
The WWF at roof B to D at 2 to 5 elevated to center of concrete during placement. Reinforcing steel was checked for
grade, size, lap, piacement and clearances where appiicable. Reinforcing steei was secured using tie wire and
supported on concrete dobie blocks. Reinforcing steel was placed in accordance with latest revisions dated 5/01/08.
Performed visual inspection during piacement of concrete in the locations listed above. Concrete was placed by boom
pump and mechanically consolidated. Cast one (1) set of four (4) (4x8) concrete test cylinders. Contractor was asked
to notify our office when additional services are needed.

Performed continuous inspection during the installation of #5 epoxied rebar dowels in the following location: (38) #5x77"
dowels at gridlines FA to G at 5.5 to 6. Verified hoies to be 3/4" in diameter by 9" in depth. Prior to installation the
predrilled holes were cleaned using a nyion brush and compressed air. Verified minimum embedment depth of 9
inches. The above listed epoxy anchors were installed using Hilti Hit-RE 500-SD epoxy with ICC# 241382. The epoxy
anchors were installed in accordance with the latest revisions dated RFI #30.
