Performed inspection' on the :reinforcing steel' for north down spout site wall.
Reinforcing steel was .checked for size', grade, laps,' quantities, locations" ties,
cle~rarice and supports.

Observed the placement of 3.5 yards of Eugene Sand and Gravel 3000 psi concrete
(mix 313065) into area noted above. Concrete was placed directly from truck and was
mechanically consolidated. Cast one (1) set of four (4) 4" x 8" concrete test cylinders.
Notified Adam Hughes (Brown Contracting) of on-site results: slump- 4 1/4"; concrele
temperature- 62F; air temperature- 38F.
Mr~ Hughes was asked to notify our office when additional services are needed.
To the best of our knowledge, the work inspected was in accordance with ANG
Engineering drawings dated 08-11-08, plans dated 03-12-D8 and applicable
workmi3nship provisions of the LB. C., except if noted abovr.

Performed visual inspection on eighty-three (83) 100" long 18 gauge roof deck units puddle welded to 16
gauge steel straps. Completed welds were checked for size and location.
