Performed periodic visual inspection of the following locations: Flare bevel weldments for chord tie at panel connections
for 2nd floor and roof elevations ,at gridlines A to D from 5 to 8. Also performed periodic visual inspection of angle to
embeds at A at 8, B at 8, C at 8, D at 8 and A at 5 except for missing embed at 2nd floor elevation at A at 8 and knife
. plate to embeds at gridlines A to D from A to 8 for 2nd.floor and roof elevations except that knife plate located at B at 5
roof elevation not welded. Fit-up of weld joints were inspected for workmanship. In-progress welding was monitored to
fabricators welding procedure specifications. Weldments were inspected for size, length, profile and locations. To the
best of our knowledge, the work inspected is in conformance with approved drawings, specifications and appiicable
AWS D1.1 codes. Contractor was asked to notify our office when additional services are needed.

. Inspection was made on completed weldments in the kitchen classroom wing at panel cord splice and beam clip to
embed and corner clip connection from grid line A to D from 2 to 8. Also work is in-progress welding cord splice
connection in the gym classroom wing from gridline 2 to 8 from E to H. In the administration wing entry and library
corner clip connection weldments have been accepted located at gridlines M at 6 in lue of miss-alignment per RF1137.

Field Observations and Comments
Performed visual inspection of reinforcing steel at the following locations: foundation wall along gridline L from 5 to 6
(RFP 38-T). Reinforcing steel was checked for grade, size, lap, placement and clearances where applicable.
Reinforcing steel was secured using tie wire and supported on concrete dobie blocks. Contractor was asked to notify
our office when additional services are needed. To the best of our knowledge, the work inspected above was in
accordance with the building department approved design 'drawings, specifications arid applicable workmanship
provisions of the International BuildinG Codes.

Reinforcing steel was perviously inspected on 11/10/2008. Performed visual inspection during placement of concrete in
the following locations: wall pour between grid 5 and 6 on line L. Concrete was placed by truck and pump trucks and
hand screeded consolidated with vibratory methods. Cast one (1) set offour (4) (4x8) concrete test cylinders. Contractor
was asked to notify our office when additional services are needed. To the best of our knowledge, the work inspected
above was in accordance with the building department approved design drawings, specifications and applicable
workmanship provisions of the International Buiiding Code.

. Performed visual inspection of reinforcing steel at the following locations: pour back strip reinforcing at the following,
gridlines J to M at 5 to 8, E to H at 1 to 5, G.5 to H at 2 to 5 and G to G.7 at 5 to 5.1 prior to placement of concrete.
Reinforcing steel was checked for grade, size, lap, placement and clearances where applicable. Reinforcing steel was
secured using tie wire and supported on concrete dobie blocks. Reinforcing steel was placed in accordance with latest
revisions dated 6/12/08. Performed visual inspection during placement of concrete in the locations listed above.
Concrete was placed by boom pump and mechanically consolidated. Cast one.(1) set of four (4) (4x8) concrete test
cylinders. Contractor was asked to notify our office when additional services are needed.

Performed periodic visual inspection of the following items: Flare bevel weldments for chord splices at 2nd floor and roof
elevations in area 2, from E to Hand 5 to 8. Also performed periodic visual inspection of "in-progress" welding for beam
to clip, beam to column, and angle to embeds at corner panel connections for 2nd ~oor and roof elevations from E to H
at 5 to 8. Fit-up of weld joints were inspected for workmanship. In-progress welding was monitored to the erectors
FCAW welding procedure specifications. Weldments were inspected for size, length, pro~le and locations. To the best
of our knowledge, the work inspected is in conformance with the approved drawings, specifications and applicable AWS

predrilled holes were cleaned using a nylon brush and compressed air. Verified minimum embedment depth of 6" inches
for trash enclosure and 9" for stairs in area 2. The above listed epoxy anchors were installed using Hilti RE500 SD
epoxy with ICC 2322 expo 2/09. The epoxy anchors were installed in accordance with the approved job-site plans and
manufacturer's directions. Contractor was asked to notify our office when additional services are needed. To the best 01

The epoxy anchors were instailed in accordance with the RFi #154 and latest revisions dated 1 H24/200B. Contractor

Reinforcing steel was previously inspected on 10106/08. Performed visual inspection during placement of concrete in the
following locations: tilt up wall panels #37,36,35,26,31,22,58,41 and 14. Concrete was placed by boom pump and
mechanically consolidated. Cast two (2) set's of four (4) (4x8) concrete test cylinders. Contractor was asked to notify our
office when additional services are needed. To the best of our knowledge, the work inspected above was in accordance
with the building department approved design drawings, specifications and applicable workmanship provisions of the
International Building Code.

'Performed visual inspection of reinforcing steel at the following locations: tilt up wall panels #46, 45, 44, 52, 43, 42, 38,
57 and 56. The required .addtional embeds was placed'in all panels listed above. Reinforcing steel was checked for
grade, size, lap, placement and clearances where applicable. Reinforcing steel was secured using tie wire and
supported on plastic chairs. Reinforcing 'I steel was placed in accordance with latest revisions dated 5/01/2008.
Performed visual inspection during placement of concrete in the iocations listed above. Concrete was placed by boom
pump and mechancially consolidated. Cast two (2) set's of four (4) (4x8) concrete test cylinders. Contractor was asked
to notify our office when additional ~ervices "are needed. To the best of our knowledge, the work inspected above was in
accordance with the building department' approved design drawings, specifications and applicable workmanship
provisions of the International Building Code.
