TEAM @J Minutes of the Joint Elected Officials' Meeting July 23, 2007 ,.- RiverStone Clinic 1640 G Street Springfield, OR 97477 The TEAM Springfield Joint Elected Officials met on Monday, July 23, 2007, at 5:30 p.m., at the Community Health Centers of Lane County RiverStone Clinic, 1640 G Street, Springfield, Oregon. ATTENDANCE City of Springfield: Mayor Sid Leiken; Councilors John Woodrow, Joe Pishioneri, Dave Ralston, and Hillary Wylie. City Manager Gino Grimaldi; Assistant Manager Jeff Towery. Springfield School District: Board Members Laurie Adams, Jonathan Light,and Bill Medford. Superintendent Nancy Golden. . Springfield Utility Board: Board Member Bobbie Adams. General Manager Bob Linahan. Willamalane Park and Recreation District: Board Members Danelle Ralston, and Helen Wagner. Superintendent Bob Keefer. Others in attendance: Steve Manela, Community Health Centers of Lane County Director; Dr. Larry Myers; Meadow Martell, Interim RiverStone Clinic Manager ad CHCLC Consultant; Toni Berube, Behavioral Health Specialist; Katy Bloch, Development Directors; and Selene Jaramillo, Programs Operations Manager and Tour Leader. INTRODUCTIONS AND WELCOME Springfield Public Schools Superintendent Nancy Golden called the meeting to order and welcomed TEAM Springfield's Joint Elected Officials and guests to the RiverStone Clinic. Introductions were made. Nancy shared background information about TEAM Springfield: · With a mission "to facilitate intergovernmental communication within Springfield public agencies and to attain the benefits and synergy of coordinated and cooperative efforts on issues or projects of mutual interest and concern," TEAM Springfield connects the City of Springfield, Springfield Public Schools, Springfield Utility Board, and Willamalane Park and Recreation District. · Ambitiously strives for safe neighborhoods, a healthy environment, and a quality education for their children. · Strategic Planning for the coming year . Director Steve Manela provided background on Community Health Centers of Lane County: One of four HSC programs (housing and human services program; CHCLC; Energy Assistance; Vets) A Partnership Between the City of Springfield, Springfield Public Schools, Willamalane, and Springfield Utility Board . . . . Team Springfield Minutes July 23,2007 Page 2 CHCLC Opened Doors in January 2004 Nine Sifes in Springfield/Eugene serving Lane County's eights sites: RiverStone; Springfield Schools Health Center; Safe & Sound at New Roads, Station 7 and Opportunity Center; Lane County Mental Health & HIV Alliance; Dental program at LCC 40.75 FTE - nearly half bilingual $5.4.mi/lion Budget Served more than 7500 patients with nearly 19,000 visits last year 95 % people served under 200% FPL; more than 1200 homeless; 26%+ Spanish speaking Program areas: · Primary care · Integrated behavioral health · Medications · Family Planning · Homeless · Farm workers · Outreach and Enrollment Nancy provided an overview of the needs in the Springfield Community: RiverStone Clinic is located in the geographic center of the Eugene/Springfield metropolitan area. The service area targets six contiguous, extremely impoverished urban census tracts in the city of Springfield. The total population in the service area is 30,395 and has one of the highest concentrations of poverty, Hispanics, and migrant and seasonal farm workers in Lane County. · 35% of service area residents without health insurance · compared with the rest of the county, residents of the target area have higher poverty levels · higher percent of uninsured residents · lower median income, and · fewer high school graduates Some of the health disparities addressed in the target area include: · high incidence of inadequate prenatal care, almost double the state rate · high teen pregnancy rate (almost double the state rate) · an immunization rate (76.1 %) of two year olds that is less than the state rate · a teen suicide rate that is the highest in Oregon · higher rate of cancer deaths per 100,000 (216.9) than the state (204.3) · disproportionately high rate of Type 2 diabetes within the Latino population Steve Manela shared information about the two Springfield sites: Springfield Schools Health Center - All Springfield students and their families have access to healthcare . · CHCLC/SPS Partnership includes financial and in-kind support · Served more than 1200 students and their families last year with over 2,000 visits; 27% Latino · Five events at the schools as well as ongoing participation in school programs such as Kindergarten orientation, parent conference nights, Family Resource Center events, etc RiverStone Clinic - Medical home to more than 12,000 patients. Last year saw nearly 6,000 patients with about 16,000 visits. Selene Jaramillo, Programs Operations Manager, lead the group on a tour of the facility. Dr. Larry Meyers shared information about the clinic. The Medical Director oversees clinics including . . . . Team Springfield Minutes July 23, 2007 Page 3 RiverStone, Springfield Schools Health Center and Safe & Sound as well as medical aspects of other sites. There are currently two MDs and one vacant position; five NPs and two vacant positions. Toni Berube, Behavioral Health Specialist, provided an overview of Integrated Behavioral Health and provided information on: · team (care coordinator, front and back staff, occupational health staff) · numbers and themes · groups · chronic pain management The meeting and tour was concluded at approximately 6:15 p.m. Minutes Recorder: Karen Lewis, Springfield Public Schools
