On November 4, 2008 our office issued permits to your representative, Derrick Westover,
for a single family residence to be located at 1217 South 41 st Street, Springfield, Oregon.
While calculating the fees for that permit, the plan reviewh neglected to include the
correct Electrical Permit Fees based on the square footage of the proposed construction.
You were charged for 2000 square feet or less, and the totiU square footage of this project
is 2116, leaving a balance of$27.94 due for the electrical permit.
Please pay the amount due prior to requesting your final mspections for this project. I
have enclosed a prestamped envelope for your convenience if you wish to make payment
by mail, or you are welcome to make'paymeilt in person at our office. Our office hours
are 8:00 a.m, - noon and from 1:00 p.m, ~ 3:00 p,m, Monday through Friday, I sincerely
