Performed periodic visual'inspection of the following items: Canopy MK 10A deck puddle welds and button punch.. 1/2
inch puddle welds placed at every other flute transversly and 12 inch O/C longitudinally.' Also button punch at.12 inch
centers. Welds and button punch ok aS'per Rod Hansen'with Nagao Pacific. To.the best of our knowledge; lhework
inspected above was in conformance with specifications and applicable 01'.1 codes. Fabricator was.askedt6 notify' our
office when additional services are needed. .

PerIormedcontinuous visual inspection of the following items: Partial-pen weids canopy MK#10A and periodic. on fillet"
welds. As per Rod' Hansen with Nagao Pacific were angle for decking is short of tube steel no weld Is nessesary stitch.
weld is O.k. Fit-up of weld joints were inspected for workmanship. In-progr.ess welding was monitored to lhe fabricator's
FCAW welding procedure specifications. To the best of our knowledge, the work Inspected above was in conformance
with approved ship drawings dated 06-02-08, specifications and applicable D1.1 codes. Fabricator was asked to notify
our office when additional services are needed. .
