Performed inspection on the completed drop-ceiling grid installation in third floor room
number 321; second floor room numbers 213, 222 (line 4 to 6), 241, 243 and 285 thru
289, The "T" suspension system for the Armstrong 2' x 2' and 2' X 4' tiles was
anchored to 7/8" wall molding along two (2) adjoining walls with pop-rivets, BERC 2
perimeter clips were installed on the opposite walls at each "T" intersection, Support
wires are anchored to roof support members andlor roof deck, and to perimeter walls
within 8',' of wall face, Seismic struts using 1" conduit encircling a support wire attached
to roof and ceiling grids were installed at :!: 12 feet on center,
Rod Carothers (Chambers Construction) was asked to notify our office when additional
services are needed,
To the best of our knowledge, the work inspected was in accordance with approved
submittal package from Haas Contracting, Inc, dated 06-30-08, package C addendum
1 prints dated 06-18-08 and applicable workmanship provisions of the I.B,C" except if
noted above,

, placed directly from truck and was mechanically consolidated, Cast four (4) 4" x 8"
compressive strength cylinders and performed field tests on load 1 as follows: concrete
temperature 71 F; air temperature 45F; slump 3 1/2",
Dave Krull (Chaml:>ers Construction) was advised and was asked to notify our office
when additional services are needed, '

Performed inspection on reinforcing steel in continuous fountain retaining wall footings
on south side of site, Rebar was checked for size, grade, lap, spacing, quantity,
supports, ties and clearances, where applicable,
,Observed placement of 19,0 yds, of Eugene Sand and Gravel 4500 psi concrete- 3000
psi required (Mix 314568), Concrete was placed' direclly from truck and was
mechanically consolidated, Cast four (4) 4" x 8" compressive strength cylinders from
load 1 and performed field tests as follows: slump- 4 1/4"; concrete temperature- 69F;
air lemperature- 47F, Dave Krull (Chambers Construction) was advised .and was
asked to notify our office when additional services are needed,
To the best of our knowledge, the work inspected was in accordance with drawing
dated 05-16-08, email from AOR dated 10-07-08 and applicable workmanship
, provisions of the I.B,C" except if noted above,
