generator enclosure walls, Rebar was checked for size, grade, lap, quantity, spacing
and clearances, where applicable, Vertical bars are in place and anchored from
previous pour and horizontal bond beam steel in top course has been installed, Mike
Helms (Hap's Masonry) stated grout placement is scheduled for later this afternoon,
Mr. Helms was asked to notify our office when additional services are needed,
To the best of our knowledge, the work inspected was in accordance with RFI #144
dated 08-26-08 and applicable workmanship provisions of the I.B,C" except if noted
above,

Arrived on-site to perform re-inspection on the lower truss connection at line C,3/1 that was noted in FEI
report E-20813 dated 08-05-08, Fit up was re-worked to make the connection using fillet welds per plans,
Welds were checked for size, length, profile and location, While on-site witnessed truss bracing being
installed by a certified welder using FCAW process, Welding was not completed during the time of
inspection,
John Shay (Chambers Construction) was asked to notify our office when additional services are needed,
To the best of our knowledge, the work in-progress is in conformance with structural drawings and
applicable AWS D1, 1 code, except if noted above,

10-09-08: Performed inspection on the reinforcing steel in the 8" CMU generator
enclosure walls (ftg, to 4'), Rebar was checked for size, grade, lap, quantity, spacing
and clearance, where applicable, Cells were relatively clean and free of excess mortar
projections, Observed placement of 2,5 yards of Knife River 7,Osk grout. Grout was
placed by pump and was mechanically consolidated, Grout slump- 9" +; grout
temperature- 65F; air temperature- 54F, Walls were grouted solid, Three (3)
compressive strength prisms were also grouted during today's pour, Mike Helms
(Jones Masonry) scheduled the next site visit for tomorrow morning,
10-10-08: Inspected rebar in second lift of enclosure walls (4' to 8') and observed
placement of 2,5 yards of Knife River 7,0 grout by pump, Grout slump- 8" +; grout
temperature- 61 F; air temperature- 44F,
Mr. Helms was asked to notify our office when additional services are needed,
To the best of our knowledge, the work inspected was in accordance with RFI #144

Observed placement of 15,0 yds, of Eugene Sand and Gravel 4500 psi concrete- 3000
psi required (Mix 31468), Concrete was placed directly, from truck and was
mechanically consolidated, Cast four (4) 4" x 8" compressive strength cylinders from
load 1 and performed field tests as follows: slump- 3"; concrete temperature- 72F; air
temperature- 66F, Dave Krull (Chambers Construction) was advised and was asked
to notify our office when additional services are needed,

Performed inspection on the completed drop-ceiling grid installation in third floor room
numbers 320, 322, thru 325, 330 thru 340 and 364 thru 377, The 'T suspension
system for the Armstrong 2' x 2' and 2' x 4' tiles was anchored to 7/8" wall molding
along two (2) adjoining walls with pop-rivets, BERC 2 perimeter clips were installed on
the opposite walls at each 'T intersection, Support wires are anchored to roof support
members and/or roof deck, and to perimeter walls within 8" .of wall face, Seismic struts
using 1" conduit encircling a support wire attached to roof and ceiling grids were
installed at:!: 12'feet on center.
John Shay (Chambers Construction) was asked to notify our office when additional
services are needed,
To the best of our knowledge, the work inspected was in accordance with approved
submittal package from Haas Contracting, Inc, dated 06-30-08, package C addendum

Arrived on-site on 09-30-08 to perform re-inspection of the items noted in FEI report E-21312 dated 09-24-
08, All items were completed and/or repaired, except the shear tabs that were backwards at line A,8/3
which has not been repaired and the bolted connections along line 3 with four (4) bolts in lieu of five (5) is
awaiting engineer's approval. John Shay (Chambers) was notified of the above conditions.
To the best of our knowledge, the work checked is in conformance with structural drawings dated 01-29-
08, AISC manual and applicable AWS D1, 1 code, except as noted above,

Performed inspection on the completed drop-ceiling grid installation in third floor room
numbers 305, 306, 309, 310, 350, 351, 353 and 357 (except at patio door/window);
second floor room numbers 206, 218 (except at window), 219, 222 to line 6 (except at
window near line 8), 234 (except at window), 235, 236, 250, 281, 282 and 284 (except
at window), The ''T'' suspension system for-the Armstrong 2' x 2' and 2' x 4' tiles was
anchored to 7/8" wall molding along two (2).adjoining walls with pop-rivets, BERC 2
perimeter clips were installed on the opposite walls at each "r intersection, Support
'wires are anchored to roof support members and/or roof deck, and to perimeter walls
within 8" of wall face, Seismic struts using 1" conduit encircling a support wire attached
to roof and ceiling grids were installed at':!: 12 feet on center, Advised by Brett Stutz
(Chambers Construction) that seismic joints are not required in large rooms,
Rod Carothers (Chambers Construction) was asked to notify our office when additionai
services are needed,
To 'the 'best of our knowledge, the work inspected was in accordance with approved
submittal package from .Haas Contracting, Inc, dated 06-30-08, package C addendum
