Periodic Inspection: 0
Performed inspection on the reinforcing s'teel in pedestrian walkway canopy footings at
lines C26, C28, C29, C30, C31, C32, C33, C35 and C36. Reinforcing steel was
checked for size, grade, laps, quantities, locations, ties, clearance and supports.
. Anchor bolts were checked for size, quantity and embedment. Bolts were secured in-
place with templates.
Observed the placement of 40 cu/yds. of Eugene Sand and Gravel 3000 psi concrete
'(Mix 313()65) into areas noted above. Concrete was placed directly fiom truck and was
mechanically consolidated. . Cast one (1) set of four (4) 4" x 8" concrete test cyiinders.
Notified Adam Hughes (Brown Contracting)' of on-site test results. Concrete
temperature 71 F; air temperature 62F; slump 2".
. Mr. Hughes was asked to notify our office when additional services are needed.
. To the best of our knowledge, the work inspected was in accordance with plans dated
03-12-08 and appiicable ~orkmanship provisions of the I.B.C., except if noted above.

Performed inspection on the reinforcing steel in remaining pedestrian walkway canopy
footings (except at lines C37). Reinforcing steel was chlkked for size, grade, laps,
quantities, locations, ties, clearance and supports. Anchor bolts were checked for size,
quantities and embedment, bolts were secured in-place with templates.
Observed the placement of 33 cu/yds. of Eugene Sand an'd Gravel 3000 psi concrete
(Mix 313065) into areas noted above. Concrete was placed directly from truck and was
mechanically consolidated. .Cast one (1) set of four (4) 4" x 8" concrete test cyiinders,
