Field Observations-and Comments
Performed inspection during the placement of grout at the following locations: East CMU wall from footing to 4 feel.
Reinforcing steel was inspected for size, grade, lap, placement, and clearances where 'applicable, All cells were
relatively clean and mortar projections are within the limits of the Uniform Building Code, Grout was placed by pump
and mechanically consolidated, Only cells containing reinforcing steel were 'grouted solid, Empty cells were covered
with grout paper to be insulated later, To the best of our knowledge, the work inspected above was in accordance
with the building department approved design drawings, specifications and applicable workmanship provisions of the
International Building. Code, Contractor was advised of all on-site results and asked to notify our office when
additional services are needed.

Performed periodic visual inspection of single pass fillet welds on tube steel column C16, RFI 7.1 (c) ASI nO.21 dated
August,; 3, 2008. At the face of the column inside of the building top connection plate to column, The weld is not
acceptable and needs to.be repaired and or engineering approval for as built condition. Weldments were inspected for
size, length, profile and locations, To the' best of our knowledge. the work inspected is,in conformance with approved
drawings, specifications' and applicable 01,; codes, except as note above.
