Inspection of: Epoxy Anchors and
Reinforcing Steel Weather: Clear Continuous Inspection: 0
Contractor: Wildish BPNo.: Pending Periodic Inspection: 0
Performed continuous Inspection during the installation of four (4) 3/4" x 24" horizontal
stainless steel all thread anchors into existing buttress at the south east corner. Prior
to installation, the predrilled holes were cleaned with compressed air and a brush.
Verified minimum embedment of 7". Anchors 'were secured with Epcon A7 (ICC~ES
ESR-6582, expiration date 11-07-09).
Performed preliminary inspection on the reinforcing steer for slab numbers 1 and 2.
Reinforcing steel was checked for size, grade. laps, quantities, locations, ties,
clearance and supports, Note: additional slab top reinforcement at north buttress
is not in place. A final inspection will be performed prior to concrete placement.
Jake (Wildish) was asked to notify our office when additional services'are needed.
To the best of our knowledge, the work inspected was in accordance with plans dated
02/2008 and applicable workmanship provisions of the !.B.C., except if noted above.
