Field Observations and Comments
Performed continuous inspection during installation of (1 1/4") ali thread epoxiecj anchor bolts in the following locations:'
At perimeter connection to pedestal footing for stezo tank, Prior to installation the' predrilled holes were cleaned using a
nylon brush and compressed air, Verified minimum embedment depth of (36") inches. The above listed epoxy anchors
were installed using RedHead Epcon A7 epoxy with ICC# 058190. Contractor stated that nuts'would be, tightened after
epoxy has cured, The epoxy anchors were installed in accordance with the approved job-site .plans. Contractor was
asked to notify our office when addiction services are needed. To the best,of our knowiedge, the' work inspected above
was in accordance with the building department approved design drawings, specifications and applicable workmanship
provisions of the International Building Code.
