Field Observations and Comments
As requested, met orl-site with Lee Construction and Professiorlal Underground Service to observe a proof roll on the
east half of the building pad. The proof roll conducted approximately 24 inches below existing grade using a fully loaded
tandem axle dump truck weighing 50,000 pounds. Deflection (pumpirlg) and wheel tracking (rutting) occurred, indicating
a soft unstable subgrade at this time. After reviewirlg the soils report prepared by Branch Engineering dated January
14, 2008 and the project specification book, the site requires a 12 irlch structure of structural fill, over Jain by 12-18
irlches of granular fill, over lain by 2 inches of sand. In order to provide this structure the existing grade will need to be
over excavated an additional 14 inches. This additionaL excavation should remove arlY undocumented fill that was
observed in the soils report as well as remove the soft material rloted above. PSI recommends observing the subgrade
following the additional excavation and prior to placement of crushed rock.
The soils report also states that the subgrade shall be covered with a geotextile separation fabric but the specification
book does not mention this. The contractor is going to get clarification on this. Also rlote that the first 12 inches of
structural fill is supposed to be a granular fill with a nominal aggregate size of 3 inches. PSI spoke to Mr. Ron Derrick
with Branch Engineering and both PSI and Branch Engineering agree that sand (bar run) gravel' with a normal
aggregate size of 4 to 6 inches could also be used. Contractor was asked 10 notify our office when additional services
are needed.

AM: As requested, a representative of PSI met on-site with Lee Construction and Professional Underground Services to
observe exposed subgra(Je soils. Subgrade soils visually appear to be the native clay described in Branch
Engin~ering's geotechnica,l report. To a'ssist us in determining the stability of in-place soils, a standard soil probe was
used. Soil probe penetrations ranged from 6 to 8 inches in'dicating a relatively firm', but above optimum moisture

subgrade. The contractor irrmediately began placing 3"-0 rock (10 inches thick), The.area observed today was the
northwest 14. .... .
Also, a proofroll was observed on the 3"-0 materials on the eas'tY. ofth!, proposed pad.. A loaded 48,0001b tandem axle
dump Iruck was utilized for the 'proofroll. Little to no defiection was observed indicating a relatively stable subgrade, at
this time.
To the best of our knowledge, the work inspected above' was in accordance with the building department approved
design drawings, specifications, geotechnical report and applicable workmanship provisions of the International Building
Codes.

PM: Returned to the site to observe the west half but the. contractor 'had .not yet started. Scheduled to return MDnday
July 14, 2008 at 3:00pm.
