Field Observations and Comme~ts
Performed visual inspection of reinforcing steeLin the fol)owing 10catiDns: 8". CMU boiler room 2' 8" to 6' 8" from footing.
Reinforcing steel was checked for grade, 'size, lap" placement and clearances where applicable. Reinforcing steel. was
placed i~ accordance with the ap'proved 'designdrawings; details 1 and 2/04 dated 1/16/08. .AII cells were relati~ely
clean and mortar projections are within .the limits of the International Building Codes. Performed continuous inspection .'
during placement of grout'in the locations listed above. Grout was placed by pump arid inechanically.consDlidated', All
cells grouted solid. Contractor was asked.to'notify our office when additional services are needed.. To the best of our
knowledge, 'the work inspected above was in accordance with' the. building department approved design drawings,
specifications' and applicable wDrkmanship provision's of the International Building Codes. " . .

Field Observations and Comments
Performed visual inspection of reinforcing steel in the following locations: 8" CMU exterior wall from top of footing to 2'
10".and top of slab to l' 4" at interiDr wall. Reinforcing steel was checked for grade,'size, lap, placement and clearances
where applicable. Reinforcing steel was placed.in accordance with .the approved design drawings; details 1 and 2/04
dated .1/16/08 (see comments). All cells were relatively clean and mortar projections are within the . limits of the
International Building Codes. 'Performed continuous inspection. during placement of. grout in the locations listed above. .
Grout was placed. by pump and mechanically consolidated..AII cells grouted solid. Additional Comments:
Specifications call out (2) #5's at bond beams, corners, and door jamb. (2).#4's placed as per' detail at bond.
beams, additional #4's placed at corners and jamb. Need Engineering approval for bond beam rebar. Contractor
was asked to notify our office when additional services are needed. To the best of our knowledge, the work inspected
above was in accordance with the building department approved design drawings, specifications and applicable
workmanship provisions of the International Buildin'g Codes, except as noted.

Performed visual inspection during the placement of concrete in the following locations: footings for boiler room building
on south east corner of school. Concrete' was placed by pump and mechanically consolidated. Cast one (1) set of three
(3) (4x8)'concrete test cylinders.. Conlractor was aSKed to notify'our office when additional services are needed. -To the
