Narrative explaining the proposal and any additional information that may have a bearing
in determining the action to be taken, including findings demonstrating compliance with
the Standards for Hazardous Materials within Time of Travel Zones (TOTZ) listed in SDC
3.3-235.
NOTE: Before the Director can approve a DWP Overlay District Development request,
information submitted by the applicant must adequately support the request. All of the
Standards for Hazardous Materials within TOTZ must be addressed. Incomplete
applications, as well as insufficient or unclear data, will delay the application review
process and may result in denial. In certain circumstances, it is advisable to hire a
professional planner or land use attorney to prepare the required findings.
Three (3) Copies of a Plot Plan reduced to 8 112" by 11" outlining the applicable TOTZ
boundaries in relationship to the property boundaries
Three (3) Copies of the Following:
o Use of hazardous materials that pose a risk to groundwater. that existed on the
property prior to May 15, 2000, if applicable
o Hazardous Material Inventory Statement and Material Safety Data Sheet for all
materials entered on the Statement. Hazardous material weights shall be converted to
volume measurement for purposes of determining amounts (i.e. 10 pounds shall be
considered equal to one gallon)
o List of chemicals to be monitored through the analysis of groundwater samples and a
monitoring schedule if groundwater monitoring is anticipated to be required
o Detailed description of the activities conducted at the facility that involve the storage,
handling, treatment, use, or production of hazardous materials in aggregate quantities
greater than 500 gallons
o Description of the primary and any secondary containment devices proposed, and, if
applicable, clearly identified as to whether the devices will drain to the sanitary sewer
or stormwater system
o Proposed Hazardous Material Management Plan for the facility that indicates
procedures to be followed to prevent, control, collect, and dispose of any unauthorized
release of a hazardous material

Proiect Overview
Hamilton Construction, Inc. is a construction company that specializes in bridge and
railway construction. They have been located at 2213 South' F' Street in Springfield
since 1953, with the site being approximately 18.2 acres in size. They are a large
employer with more than 100 employees (most in the field), with about 25 employees at
the South' F' Street site. Due to the complexity of their projects, much of the parts for
bridge and railway construction are pre-fabricated at this location and delivered to their
various job sites.
The current site is gravel due to the industrial nature of the operation. The existing
conditions plan shows that the majority of the property is being used for material storage,
with the western buildings on the site being used as office and pre-fabrication.
The current fabrication process involves the (2) mos1 western buildings and the western
portion of the office building. This proposal is to construct a new 6000 square foot
fabrication building which will consolidate to operation.
As shown on the site plan, the new building will be southeast of the existing fabrication
building, connected with a new concrete ramp. This will eliminate the current internal
traffic pattern for forklifts and utility vehicles driving through the yard to other buildings.
This will provide (3) benefits to the operation:
I. Internal fabrication operation will be more efficient.
2. Reducing utility vehicle travel through the yard is safer.
3. Reducing utility travel on the gravel surface will improve air quality.
Upon completion of this new fabrication building, Hamilton Construction, Inc. is proposing to
remove the existing secure storage building (shown as phase 2 on the site plan).
. Time of Travel Zone
The site sits within the 10-20 year time of travel zone as per the Wellhead Protection Map.
Plan of Action
The Hamilton Construction site has many existing facilities, that have been in place for many
decades. Below is a summary of the items that were discussed with Mark Metzger and Amy
Chinitz in regards to improvements to the site and their timing. A specific Hazardous Material
Management Plan (HMMP) has been prepared, and (3) copies are attached to this submittal.
Improvements Proposed During This Construction Phase
I. A Hazardous Material Management Plan (HMMP) has been prepared (attached).
2. Spill protection will be provided for all hazardous material storage areas. See attached
HMMP for detailed description:
3. Personnel will be trained on the elements of the HMMP and what to do in case ofa
spill.
4. Monthly inspection of the hazardous material will be conducted and documented.
. When a compliance issue is identified, a corrective action will be performed and
documented.
Future Improvemeuts
As future phases are developed on the site, additional protection measures will be installed. They will
include:
I. Paving of key areas ofthe site to prevent any spills to enter into the ground.
2.. There will be a defined truck wash area installed to isolated the use of washing agents,
complete with a drain connected to the sanitary system
3. Dumpster areas will be upgraded to be hydraulically isolated and include drains to the
sanitary system.
The timing of these measures are not presently known, and will depend on economic conditions
and company plans for future improvements to the site.

The Hazardous Material Management Plan (HMMP) for Ham'ilton Construction Company (HCo)
located in Springfield, Oregon serves several objectives. Ii

The plan establishes responsibilities, policies, prevention me:~sures, and procedures for the
storage and usage of hazardous materials at Hamilton CordriJction's facility Requirements'for
hazardous materials have been established by the City of Sp'ringfield Drinking Water Protection
(DWP) Overlay District and rely on the Uniform Fire Code (UFC). One purpose of the plan is to.
meet DWP requirements and .protect the groundwater used ~s a city drinking water supply from
contamination by discharges of hazardous material that po.s~ a risk to groundwater. The plan
addresses the following hazardous material management processes (the applicable section of the
Plan is identified in parentheses): Ii

(TOTZ), restrictions apply to hazardous materials to protect groundwater. The Springfield
Development Code defines hazardous materials as those substances (liquid or solid)
which pose a risk to groundwater quality (Chapter 3.3,205 A.1.). The class of hazara.:lus
materials is based on Article 80 of the UFC, which ha~ been as adopted and amended by
the City, including materials in both usable or waste conditions.
City of Springfield Land Use Development Code, Chabter 3.3-230exempts several ty::!es
of UFC hazardous materials, these exceptions include: (1) Hazardous materials in fuel
tanks and fluid reservoirs attached to (i) a private or commercial motor vehicle and used
directly in the motoring operation of that vehicle, or (ii): machinery, including but not limi,ed
to fuel, engine oil and coolant; (2) Fuel oil used in existing heating systems; (3) Hazardous
materials contained in properly operating sealed units (transformers, refrigeration units,
etc) that are not opened as part of routine use; (4) Local natural gas distribution lines; (5)

Fuel for emergency generators located at facilities that provide essential community
services (hospitals, fire/life safety, police, public shelters, telephone systems, etc.); and (6)
Any commonly used office supply - such as correcting fluid for typewriters, toner for
computer printers or cleaners for windows bathrooms- where the supplies are purchased
. for use on-site.
3.2 Hazardous Material Inventory Statement
The Facility has developed a comprehensive Hazardous Material Inventory Statement
(HMIS) that lists all hazardous materials used at the Facility, maximum quantity, and
storage location within the property limits. This list accompanies this plan and is located in
Appendix A.
3.3 Hazardous Material Storage Locations
The hazardous material storage locations are listed in the HMISin Appendix A.
A site drawing depicting storage locations is attached :'in Appendix B.
3.4 Hazardous Material Management
. All hazardous materials defined above will be stored, handled, treated, or used at the
Facility according to the guidelines outlined in this Plan. Hazardous materials will be .

purchased only where necessary and used only for the intended purpose. Hazardous
materials will only be stored in designated areas with appropriate secondary containment.
Hazardous materials will be used outdoors with precautions to prevent hazardous material
,from entering stormwater or infiltrating into the ground. Facility personnel will take.
precautions to prevent releases of hazardous materials during storage and use, and any
releases that do occur will be controlled, cleaned up promptly, and disposed of.

.5 Signs will be posted outdoors in the parking lot, on structure walls and the fuel storage
.~ area. The signs will advise personnel that the Facility is located in a Wellhead Protection
o Zone and will alert staff and visitors to notify 911 in the event of unauthorized release of
hazardous materials.
Driving and parking area's are not paved at the Facility therefore regularly monitoring
parked vehicles and equipment for oil and fluid leaks on site is important. If equipment
or vehicles are found to be leaking, drip pans will be placed under the vehicle to prevent

ground contamination until the vehicle can be properly ser'/iced and repaired.
Drip pans shall be used at the fueling area when vehicles ,ore fueled to catch overflow
and unanticipated drips and leaks. Flammable material safety cabinets will be used
inside buildings for small amounts of hazardous mat~rials that are used on a day to day
basis.
The Facility will utilize a Service Contractor for janitorial services. All janitorial products will
be maintained and used only by the janitorial service. The products will be used at the
Facility for short periods and will not be stored at the Facility.
In addition, all chemical products intended for use by a Service Contractor (i.e.,
janitorial service or landscaping service) will be reviewed by Hamilton Construction's
Safety Manager prior to being brought on site.
Hamilton Const;~ction will upda;~ the HMIS when new hazard~us materials are
added or removed from the inventory, and will provide copies of the updated HMIS
to the City of Springfield as necessary.
4.0 HAZARDOUS MATERIAL SECONDARY CONTAINMENT AND SPILL PREVENTION.
Hazardous materials will be stored, handled, and used with" precautions to avoid spills.
4.1 Secondary Containment
The City of Springfield requires that all hazardous material containers must have
secondary containment regardless of type, quantity, or size. Secondary containment may
be provided by one of the following means:'
. Liquid-tight sloped or recessed floors;
. Liquid-tight floors with raised or recessed sills or dikes;
. Sumps and collection systems;
. Drainage systems leading to a controlled location; or
. .. ... -
. Secondary containment devices, such as spill pallets, decks, tubs, or trays for
containers.
Secondary containment systems will be sized to accommodate a spill from the largest
container. The materials of construction of the secondary containment.system must be
compatible with the hazardous materials to be stored, Incompatible hazardous materials
will be separated from each other in the secondary containment system. The secondary
containment system will have a monitoring method, such as visual observation. .
The hazardous materials to be stored at this Facility include hazardous petroleum
products, paints, solvents, compressed gasses, and cutting oil. Hazardous paint products
and joint seal products are sometimes temporarily collected here from mobile work sites
for disposal (these items are not included in the HMIS). The secondary containment
devices will allow easy access and quick inspection to determine if any release of
hazardous materials has occurred.
The primary form of secondary containment at this Facility will be spill basins, spill
pallets,and other like devices. .

4.2 Spill Prevention
In addition to secondary containment, Hamilton persor,el will follow precautions in
hazardous material storage and use to prevent spills. rile following measures will be
used for spill prevention
. . Containers of hazardous materials will be storee II; various locations throughout the
Facility in flammable material safety cabinets. Sate storage and use of hazardous
materials will only occur in conjunction with Facility safety.procedures.
. All containment areas and hazardous material containers will be covered, either by
building with a roof or by covering containers to prevent rainfall to flood and overflow
the containers or secondary containment devices.
. Hazardous materials should not be left in vehicular traffic areas or any other
unrestricted or unprotected locations.
. All containers of hazardous materials will be labefed with the identity of contents.
. Hazardous material containers must be in good condition, compatiblewith the
material stored therein, and not in danger of leaking. Containers should be kept
closed except when the material is in use. Non-compatible materials should not be
stored next to each other.
. Containers stored in cabinets, closets and on-shelves should be at eye-level or below.
. Storage areas should be well-lit. Hazardous material storage areas should be
secured or locked, if appropriate, to protect against unauthorized access..
o Hazardous material storage areas should have signs to provide safety information
and spill cleanup procedures. .. ..
. Spill cleanup kits and absorbent materials will be readily available near hazardous
material storage areas.

o Appropriate spill absorbent materials should be readily accessible for placement in
order to contain a material spill or release. It is crucial that all areas with unprotected
floor/ground surfaces are protected during such spill and release events.

5.0 CHEMICAL EMERGENCY/SPILL RESPONSE
5.1 General
Personnel who work with or around chemicals will be trained in these procedures.
Hazardous material container storage areas will be managed to minimize the possibility of
fire, explosion, or any unplanned release of chemicals to the environment.
5.2 Emergency Requirements.
Hamilton Construction maintains a Facility Emergency Plan, located in Appendix C,
which includes the following elements required for hazardous material management:
. Communications and fire control equipment', including periodic inspection to.
ensure equipment will operate properly in time of emergency. .
. Arrangements with local authorities or Hazardous Material Companies.
. The Emergency Coordinator for the Facility who has been designated with the
responsibility for coordinating a[1 emergency response measures.
5.3 SPILL RESPONSE PROCEDURES:
The level of response to a hazardous material spill will depend upon the physical
characteristics and volume of materials being handled, their potential hazards, and the
potential for releases to the environment. The following are general guidelines to be
followed for a hazardous material spill:
a. The individual who caused or discovers the spill is responsible for
immediate response and containment until the Emergency Coordinator
arrives.
b. Immediately alert area occupants and supervisor, and evacuate the area,
if necessary.
c. If a volatile, flammatii'e material is spilled, immediately warn everyone,
control sources of ignition and ventilate the area.
d. Attempt to contain the spill (with absorbent material) to the smallest
practical area.
e. Wear. personal protective equipment, as appropriate to the hazards. Refer
to the Material Safety Data Sheet (MSDS) fOr appropriate personal
protective equipment..
f When spilled materials have been absorbed, the absorbent must be
placed in an appropriate container, which will be located with all spill kits.
g. For.spills of hazardous material, the waste product and spill control
materials might be classified as a hazardous waste. For a spill of
hazardous material, label the container to identify the material as Spill
Material Involving "XYZ Chemical" and place the used materials in the
covered storage area for disposal.
h. Decontaminate the surface where the spill occurred using a mild
detergent and water, when appropriate
i. If a spill cannot be contained by the spill kits or other means readily
available, call NortWest HazMat spill response 24 hours a day at,
1-800-597 -1323.
J. As appropriate, spills should be cleaned up by chemical treatment,
absorption, or ~eutralization. If the spill is too large for the responsible

5.4 REPORTING PROCEDURES:
a. The Emergency Coordinatorwill complete or assign necessary reporting
to authorities and agencies. The Emergency Coordinator will assess the
need for any required environmental reporting.
b. The following quantities of hazardous materials will be reported as soon
as practical.
'. Any amount of oil or petroleum product which spills into the waters
of the state, such as the Millrace located behind the property.
. Any oil or petroleum product in 'excess of 42 gallons.
... Any hazardous material that are equal to or greater than;'the
quantity listed in the Code of Federal Regulations, 40 CFR Part
302 (List of Hazardous Substances and Reportable Quantities,)
an,d amendments adopted before July 1, 2002, .
6.0 PERSONNEL TRAINING FOR HAZARDOUS MATERIAL MANAGEMENT
In general, Facility personnel will receive direction or training from their supervisors, as necessary
for each position, in hazardous material management General training given to all employees in
their group does not need to be documented. .
Facility personnel who handle hazardous materials may need basictraining and direction if
their position has responsibility for:.
. Awareness of the sensitive area that the Facility occupies and special precautions
needed to protect the. drinking water.
. .General hazardous.material storage and use guidelines.
. Responding to spills o(other hazardous material related emergencies,
. . , .
At a minimum, all Facility staff will be trained on the following: HMMP requirements, Emergency
Plan procedures, and DWP. awareness.
7.0 MONTHLY INSPECTIONS OF HMMP AND CORRECTIVE ACTION SYSTEM
Monthly inspections of hazardous material storage areas will be conducted. This Facility is
unlikely to generate regulated hazardous waste at this time, therefore, only the hazardous

FACILITY EMERGENCY PLAN
In the unlikely event of a Hazardous Materials spill or emergency, quick response
is surest way to limit the amount of a spill, damage,or injury.
The level of response or clean up effort will depend on the amount and type of
material spilled. The emergency coordinator will have final responsibility to
ensure that a Hazardous Materials spill has been contained and disposed of in
accordance with this plan.
Monthly Inspections: A monthly inspection of all Hazardous Materials
storage locations will be carried out by the authorized person. The inspection will
include the condition of any containers, secondary containment, spill kits, and
portable fire extinguishers. The inspection will note any problems with leaking
cohtainers, broken or damaged secondary containment, deficient spill kits, and
deficient, outdated, or missing fire extinguishers. If there is a problem noted on
the inspection form, the emergency coordinator will ensure that the deficiency is
corrected.
Initial Response: The person who creates or discovers a chemical spill has
the responsibility to identify the type and amount of material spilled, and attempt
to containit. This person shall also see that the emergency coordinator or other
personnel are notified. If responding to an accident, inform someone in the office
of the nature and extent of the injury or damage so that they can contact the
appropriate agencies.
.Communications: All yard employees and staff are equipped with two-way
. radios, fabrication shop employees generally are not. If a yard employee
discovers a spill or responds to an accident, they'will radio in to the office as
soon'as practical to identify the type and extent of spill or accident. It is extremely
important that the person who discovers and reports an incident remains calm
andrelays all pertinent information as clearly as possible. See the instructions
below:
1. Identify yourself'
2. Identify the type of emergency, i.e injury, spill, fire, etc.
3. If an injury has occurred, relay the type and extent of the.injury, i.e. cut,
laceration, burn: etc. and the area of the body, head, arms, legs, chest,
etc.
4. State if assistance is needed i.e. NorthWest HazMat, Med Express,
Ambulance, Fire or Police Department, etc.

Spill Agreement: Hamilton Construction has a "Spill Agreement" with
NorthWest HazMat to coordinate and respond to any spill that cannot be properly
handled "inohouse". If NorthWest HazMat is called to respond to a spill on the
property, they will be:responsible io report to all appropriate agencies and
authorities, the nature, type, and amount of.contaminant spilled or released. See
the attached Spill Agreement, and Spill Response Flow Chart.
. Emergency Evacuation:. .In the event of a fire, explosion, or uncontrolled
release of chemicals, and if it is deemed necessary to evacuate the work areas
of the property, personnel should evacuate by the safest route possible to the
front of the main office on South F Street. During an evacuation, supervisors will
account for all employees and try to determine the location of.ernployees.still on
the property. Employees will remain there until it is ascertained that it is safe to
occupy the work 'area again. That determination will be made by the emergency
coordinator, alternate, or responding agency, whoever has authority

SERVICE AGRlEEMlENT
This service agreement dated 09-22-2008, is between NorthWest HazMat, Inc. and Hamilton
Construction Company, 2213 South "F" Street, Springfield, Oregon 97477 and Other
Construction Locations for future services by NorthWest HazMat, Inc.
When called, North West HazMat can provide response services as ali independent contractor or
provide. response. services through our Spill Responder Network@. NorthWest HazMat or a Spill
Responder Contractor will respond promptly to control; contain, clean up; transport; and dispose of
. all spilled material, contaminated material, or hazardous waste. In compliance with state and federal
regulations, NorthWest HazMat, Inc. will insure all necessary reports and notifications are made.
If the job is accepted, the initial response shall be made by a minimum of one (l)supervisor and one.
(I) hazardous materials technicians in addition to any equipment needed to complete the work
assignment. NorthWest HazMat shall be given full responsibility and authority at the scene of all
incidents on behalf of, and in the best interest of the cI ient. The cI ient agrees to cooperate with
NorthWest HazMat regarding services requested or required. NorthWest HazMat agrees to provide
services in a safe and cost effective manner.
The client agrees that labor and fees furnished by North West HazMat or a Spill Responder Contractor
is for time and materials at the current rate schedule. The client agrees to pay NorthWest HazMat
within thirty days ofthe invoice date. Invoices from Spill Responder Contractors are reviewed and
processed through North West HazMat, Inc.
North West HazMat agrees to carry and maintain all required insurance coverage. . Proof of insurance
is available on request. This agreement and all other documentation and conversations related to this
agreement are confidentiaL The client reserves the right to terminate (in writing) the services of
North West HazMat at any time during an incident if not completely satisfied with their performance.
The client is obligated to pay up to the time notice is given. NorthWest HazMat requires no retainer
fee for this service agreement. . .
Through this agreement, client, if a transporter/shipper is permitted to list the North West HazMat,
Inc. emergency response telephone number as a point of conta~t on shipping papers, air bills,
