ALL WORK & MATERIALS SHALL CONFORM TO CURRENT OREGON U.P,C., 2002 STATE OF OREGON APWA I ODOT STANDARD SPECIFICATIONS FOR CONSTRUCTION, & ALL APPLICABLE STATE, CITY, AND COUNTY REGULATIONS AND STANDARDS. CONTACT ENGINEER FOR DIRECTIVE IN THE EVENT OF CONFLICTING STANDARDS. 2. ALL WORK WITHIN THE PUBLIC RIGHT-OF-WAY SHALL BE COORDINATED WITH THE GOVERNING AGENCIES INSPECTOR AND SHALL CONFORM TO THAT AGENCIES CURRENT ENGINEERING STANDARD SPECIFICATIONS & DETAILS. 3. IT SHALL BE THE RESPONSIBILITY OF THE CONTRACTOR TO IDENTIFY ALL UTILITIES AND PROTECT AS REQUIRED DURING THE COURSE OF CONSTRUCTION. 6. ALL SITE CONCRETE SHALL BE fe = 3500 psi @ 28 DAYS, 6% ENTRAINED AIR, 4" SLUMP, U.N.O. 9. ALL UTILITY SERVICES SHALL BE INSTALLED PER THE RESPECTIVE UTILITY CODES & STANDARDS. 10. ALL UTILITIES SHALL HAVE A MINIMUM COVER OF 30" UNLESS OTHERWISE SPECIFIED. 11. ALL SERVICES SHALL BE ADEQUATELY MARKED AS REQ'D SO AS TO IDENTIFY THE SIZE, TYPE, & DEPTH OF THE SERVICE. 12. ALL SERVICES SHALL BE PLUGGED AS REQUIRED TO ADEQUATELY ENSURE THAT NO FOREIGN MATERIALS ENTER THE LINE. 13. CONTRACTOR SHALL OBTAIN ALL APPLICABLE PERMITS PRIOR TO CONSTRUCTION. 14. ALL SANITARY SEWER WASTE LINES SHALL BE PVC GASKET JOINT GRAVITY SEWER PIPE CONFORMING TO ASTM 03034 - SDR 35. SEE MECHANICAL PLANS FOR WASTE LINE DIAMETER, PIPE MATERIAL, AND LAYOUT REQUIREMENTS WITHIN BUILDING FOOTPRINT. 15. SANITARY LINES SHALL BE REQUIRED TO PASS A LOW PRESSURE AIR TEST PRIOR TO FINAL ACCEPTANCE. ALL PARTS OF THE SYSTEM SHALL BE CLEANED PRIOR TO FINAL ACCEPTANCE. THE CONTRACTOR SHALL NOT ALLOW ANY FOREIGN MATERIAL TO ENTER THE EXISTING SYSTEM. THE CONTRACTOR SHALL PROVIDE THE REQUIRED PERSONNEL AND MATERIAL TO PERFORM THE ABOVE TESTS. THE CONTRACTOR SHALL PROVIDE THE ENGINEER WITH DOCUMENTATION OF THE ABOVE TESTS. 16, STORM COLLECTION SYSTEM DESIGNED FOR SILT TIGHT COMPONENTS. 17. ALL STORM PIPE IDENTIFIED AS 'HOPE' SHALL BE ADS N-12 AND PIPE IDENTIFIED AS "PVC" SHALL CONFORM TO ASTM D 3034 - SDR 35 WITH GASKET JOINTS. ALL STORM PIPE IDENTIFIED AS "CMP" SHALL BE GALV. 12 GAUGE CORRUGATED METAL, U.N.O. SEE PLAN SET FOR ADDITIONAL INFORMATION. 18. ALL STORM COLLECTION SYSTEM CONNECTIONS AND COMPONENTS SHALL CONFORM TO MFR. REQUIREMENTS. GC TO COORDINATE STORM SYSTEM LAYOUT WI ENGINEER AND STORM SYSTEM SUPPLIER AT TIME OF CONSTRUCTION. STORM SYSTEM COMPONENT SHOP DRAWINGS SHALL BE PROVIDED FOR ENGINEER'S REVIEW PRIOR TO CONSTRUCTION. 19. STORM SYSTEM CATCH BASINS SHALL BE PROVIDED WITH A MINIMUM 24" SETTLEMENT SUMP BELOW THE LOWEST PIPE INVERT & A BMP INC. SNOUT OR ENGINEER APPROVED EQUAL. SEE PLAN SET FOR ADDITIONAL INFORMATION. 20. ALL STORM DRAIN PIPE SHALL BE BEDDED IN 3/4" MINUS CRUSHED ROCK COMPACTED TO 95% MODIFIED PROCTOR. SEE PIPE MANUFACTURER INSTALLATION SPECIFICATIONS FOR ADDITIONAL REQUIREMENTS. ALL PIPE SHALL BE INSTALLED PER MANUFACTURER SPECIFICATIONS. 21. ENGINEER SHALL INSPECT ALL STORM DRAIN SYSTEMS FOR CONFORMANCE TO SUBMITTED PLANS. GC SHALL NOTIFY ENGINEER 48 hrs PRIOR TO ALL BACKFILL OPERATIONS ABOVE PIPE HAUNCH. 22. ALL LANDSCAPED AREAS SHALL BE PREPARED AS NOTED ON THE LANDSCAPING PLANS (BY OTHERS). 23. SEE LANDSCAPING PLANS FOR PLACEMENT LOCATIONS AND REQUIREMENTS FOR IRRIGATION SLEEVES. IRRIGATION SLEEVES SHALL BE PROVIDED BETWEEN EACH LANDSCAPE AREA SUFFICIENT FOR IRRIGATION AND ELECTRICAL COMPONENTS PRIOR TO FINAL PAVING ACTIVITIES. 24. ALL WATER WORKS SHALL BE DONE IN ACCORDANCE WITH THE CURRENT REQUIREMENTS OF THE SPRINGFIELD UTILITY BOARD (SUB). STANDARD LAND.& DRAINAGE ALTERATION PERMIT (LDAP) NOTES: 1. PRIOR TO ANY GROUND DISTURBANCE ON THE SITE ONE INSPECTION WITH LDAP STAFF IS REQUIRED. 2. ISSUANCE OF THIS PLAN DOES NOT RELIEVE THE PERMIT HOLDER AND OR THE CONTRACTOR FROM ALL OTHER PERMITTING REQUIREMENTS. PRIOR TO BEGINNING CONSTRUCTION ACTIVITIES, ALL OTHER NECESSARY APPROVALS SHALL BE OBTAINED. 3. THE EROSION AND SEDIMENT CONTROL MEASURES SHOWN ON THE PLAN ARE THE MINIMUM REQUIREMENTS FOR ANTICIPATED SITE CONDITIONS. DURING THE CONSTRUCTION PERIOD, THESE MEASURES SHALL BE UPGRADED AS NEEDED FOR UNEXPECTED STORM EVENTS AND TO ENSURE THAT SEDIMENT AND SEDIMENT-LADEN WATER DOES NOT LEAVE THE SITE. 4. THE IMPLEMENTATION OF THE ESCP AND THE CONSTRUCTION, MAINTENANCE, REPLACEMENT, AND UPGRADING OF THE EROSION AND SEDIMENT CONTROL MEASURES IS THE RESPONSIBILITY OF THE PERMIT HOLDER AND OR THE CONTRACTOR UNTIL ALL CONSTRUCTION IS COMPLETED AND ACCEPTED AND VEGETATION /LANDSCAPING IS ESTABLISHED. 5. THE BOUNDARIES OF THE CLEARING LIMITS SHOWN ON THIS PLAN SHALL BE CLEARLY FLAGGED IN THE FIELD BY THE ENGINEER PRIOR TO CONSTRUCTION. DURING THE CONSTRUCTION PERIOD, NO DISTURBANCE BEYOND THE FLAGGED CLEARING LIMITS SHALL BE PERMITTED. THE FLAGGING SHALL BE MAINTAINED BY THE PERMIT HOLDER AND OR THE CONTRACTOR FOR THE DURATION OF CONSTRUCTION. 6. THE EROSION AND SEDIMENT CONTROL MEASURES ON ACTIVE SITES SHALL BE INSPECTED AND MAINTAINED DAILY AND WITHIN THE 24 HOURS AFTER ANY STORM EVENT OF GREATER THAN 0.5 INCHES OF RAIN PER 24 HOUR PERIOD. MEASURES SHALL BE INSPECTED BY THE PERMIT HOLDER AND OR THE CONTRACTOR AFTER EACH RAINFALL AND AT LEAST DAILY DURING PROLONGED RAINFALL. ANY REQUIRED REPAIRS OR ADJUSTMENTS SHALL BE MADE IMMEDIATELY. THE EROSION AND SEDIMENT CONTROL MEASURES ON INACTIVE SITES SHALL BE INSPECTED A MINIMUM OF ONCE EVERY TWO (2) WEEKS OR WITHIN 48 HOURS FOLLOWING A STORM EVENT. 7. ALL EROSION AND SEDIMENT CONTROL MEASURES SHALL BE PROTECTED FROM DAMAGE AT ALL TIMES. CONTROL MEASURES SHALL REMAIN IN PLACE UNTIL PERMANENT RE-VEGETATION HAS BEEN STABILIZED. ANY MEASURE THAT IS DAMAGED OR DESTROYED SHALL BE REPAIRED OR REPLACED IMMEDIATELY. 8. ANY AREAS OF EXPOSED SOILS, INCLUDING ROADWAY EMBANKMENTS, THAT WILL NOT BE DISTURBED FOR TWO DAYS DURING THE WET SEASON (OCTOBER 1 TO APRIL 30) OR SEVEN DAYS DURING THE DRY SEASON (MAY 1 TO SEPTEMBER 30) SHALL BE IMMEDIATELY STABILIZED WITH AN APPROVED ESC METHOD (SEEDING & MULCHING WITH STRAW, BARK, COMPOST, OR PLASTIC COVERING, ECT.). 9, STREET SWEEPING SHALL BE PERFORMED AS NEEDED OR WHEN DIRECTED BY THE CITY INSPECTOR TO INSURE PUBLIC RIGHT-OFWA YS ARE KEPT CLEAN AND FREE OF DEBRIS. STREET FLUSHING IS PROHIBITED. 10. WHEN TRUCKING SATURATED SOILS FROM THE SITE, EITHER WATER-TIGHT TRUCKS SHALL BE USED OR LOADS SHALL BE DRAINED ON SITE UNTIL DRIPPING HAS BEEN REDUCED TO NO MORE THAN ONE GALLON PER HOUR. SEDIMENT LADEN WATER WILL NOT BE ALLOWED TO ENTER THE STORM WATER SYSTEM. 11. EXTRACTED GROUND WATER FROM EXCAVATED TRENCHES SHALL BE DISPOSED OF IN A SUITABLE MANNER WITHOUT DAMAGE TO ADJACENT PROPERTY, CITY'S STORM WATER SYSTEM, WATER FEATURES, AND RELATED NATURAL RESOURCES. APPROVAL OF A DEWATERING SYSTEM DOES NOT GUARANTEE THAT IT WILL MEET COMPLIANCE OR BE ACCEPTABLE FOR USE IN ALL SITUATIONS. MODIFICATIONS TO THE DEWATERING SYSTEM WILL BE REQUIRED IF COMPLIANCE CAN NOT BE MET. AT NO TIME WILL SEDIMENT LADEN WATER BE ALLOWED TO LEAVE THE CONSTRUCTION SITE. NO HAZARDOUS SUBSTANCES, SUCH AS PAINTS, THINNERS, FUELS AND OTHER CHEMICALS SHALL BE RELEASED ONTO THE SITE, ADJACENT PROPERTIES, OR INTO WATER FEATURES, THE CITY'S STORM WATER SYSTEM, OR RELATED NATURAL RESOURCES. 14. THE ESC FACILITIES SHALL BE INSPECTED DAILY BY THE CONTRACTOR AND MAINTAINED TO ENSURE CONTINUED PROPER FUNCTIONING. WRITTEN RECORDS SHALL BE KEPT OF WEEKLY REVIEWS OF THE ESC FACILITIES DURING THE WET SEASON (OCTOBER 1 TO APRIL 30) AND OF MONTHLY REVIEWS DURING THE DRY SEASON (MA Y1 TO SEPTEMBER 30). ADDITIONAL LDAP NOTES: 15. SEE GEOTECHNICAL ENGINEERING SERVICES REPORT BY PSI FOR FURTHER DETAILS. 16. CONSTRUCTION SCHEDULE/HAUL ROUTES SHALL BE PROVIDED TO THE CITY OF SPRINGFIELD BY THE GC PRIOR TO CONSTRUCTION ACTIVITIES. 17. GC SHALL PROVIDE DEDICATED CONCRETE WASH OUT AREA IF NECESSARY, ENSURING CONCRETE/SLURRY WASH OUT DOES NOT LEAVE THE SITE OR ENTER THE STORM DRAIN SYSTE. ALL CONCRETE NOT USED FOR CONSTRUCTION SHALL BE DISPOSED OF AT AN APPROVED LOCATION. 18. IF STOCKPILES OF EROSIVE MATERIALS ARE UTILIZED ON SITE DURING CONSTRUCTION, THEY SHALL BE SURROUNDED BY SEDIMENT FENCING, STRAW WATTLES, BIOFIL TER BAGS, ETC. WHEN NOT CURRENTL Y BEING UTILIZED FOR CONSTRUCTION ACTIVITIES, DURING PERIODS OF WET WEATHER, OR WHEN LOCATED IN AN AREA THAT COULD ALLOW MATERIAL TO LEAVE THE CONSTRUCTION SITE. 19. ALL IMPROVEMENTS THAT WILL BE PRIVATELY OWNED AND MAINTAINED WILL BE BOUND BY THE CURRENT REQUIREMENTS OF THE STATE OF OREGON STRUCTURAL SPECIALTY CODE, PLUMBING SPECIALTY CODE, AND/OR CITY OF SPRINGFIELD BUILDING DIVISION REQUIREMENTS. REQUESTS BY THE CONTRACTOR FOR CHANGES SHALL BE APPROVED BY THE ENGINEER AND THE CITY PRIOR TO IMPLEMENTATION. 23. CONTRACTOR SHALL SUBMIT EVIDENCE OF INSURANCE IN ACCORDANCE WITH THE STANDARD SPECIFICATION TO THE CITY FOR APPROVAL PRIOR TO BEGINNING WORK. 24.
