Performed final inspection on the reinforcing steel in the 8" CMU walls on line A, from 8
to 9, on C, from 8 to 9, on 9, from A to C (top of slab to 40"), and on 8.6, from A to C
(top of slab to 48"). Reinforcing steel was checked for grade, size, lap, placement and
clearance'. Cells were relatively clean and free of excessiv,!3 mortar projections,
Observed the piacement of 8.0 cu.lyds. of Eugene Sand and Gravel 2500 psi grout
, (Mix 312531), Grout was placed by pump and was mechanically consolidated. Grout
was sampled during placement and three (3) compressive strength prisms were
grouted. Jim Haines (Haines Bros. Masonry) was advised of on-site test results: grout
slump :1:9"; grout temperature 71 F; air temperature 70F. .
Mr. Haines was asked to notify our office when additional services,are needed.

To the best of our knowledge, the work inspected was in accordance with plans dated
10-29-07 and applicable workmanship provisions of the I.B'.C., except if noted above.

Performed preliminary inspection .on the reinforcing steel for OMS building slab on
grade. Reinforcing steel was checked for grade, size, laps, quantities, locations: ties,
supports and clearance. A final inspection will be performed prior to concrete
placement.
To the best of our knowledge, the work inspected was in 'accordance with plans dated
11-27-07 and applicable workmanship provisions of the I.B.C., except if noted above.

Performed final inspection on the reinforcing steel for OMS building slab on grade. A
preliminary inspection was performed on 04-02-08 (ref.: F~I report no. E19532).
Observed the placement of 75.0 cu.lyds. of Eugene Sand and Gravel 4000 psi
concrete (Mix 314068) in areas listed above. Concrete was placed by pump and was
mechanically consolidated. , Cast one (1) set of four (4) 4'; x 8" concrete test cylinders. .
Ian (WBC) was notified of on-site test results: slump 5 1/2'; concrete temperature 64F;
air temperature 34F.
Ian was asked to notify our. office when additional services are needed..
