. Performed continuous inspection during installation of one (1) 5/8" x 24" all thread
anchor securing added HDD5 hold down at shear wall on south side of unit 119.
Predrilled hole was cleaned using compressed air and brush. Verified 16" embedment
depth. Anchor was secured using Hilti RE-500 epoxy (ICC ESR 16821 expiration date
5/2009). Anchor 10 be left undislurbed until epoxy has set .
Kurt (Superintendent) was asked to notify our office when additional services are
needed.
To the' best of our knowledge, the work inspected was in accordance with instructions
on engineer's site visit report dated 06-12-08 and applicable workmanship provisions of
the LB.C., except if noted above.
