Springfield Inpatient Rehabilitation Hospital
In order for the stormwater treatment facilities to continue operating at acceptable levels, regular
maintenance and Inspection are required. This plan provides maintenance and Inspection
Instructions. The Operations and Maintenance Plan shall be recorded with Lane County.

Responsibility
The Facn iN is to be maintained by the property owner. The preparer has designed a system that can be easily
maintained by me maintenance staff.
A copy of the operators 6 Maintenance Flan shall be provided to all property owners and tenants.
Description
Site Description
The site Is approximately 5 acres and on the northeast corner of the Intersected of Mapel Island Road and
East Game Farm Road It will consist of the Sorire ltl Inpatient Rehabilitation Hospital (SIRH) building, a
paved parking lop paved drive aisles, paved walkways and landscaped areas.
Stormwater System Description
The Stormwater System collects, treats and Intimates runoff from pollution generating surfaces within the
site, as required bvthe cN of Sorrel
Al runoff from the Impervious surfaces atthe SIRH site will be treated by Rain Gardens. All overflow runoffs
will be piped and conveyed to a public stormwater main on Mapel Island Road west of the site.
Al runoff from the drive aisles, parking lot and walkways will be directed to Rain Gardens. All stormwater
runoff will be filtered through Growing Media and Infiltrate Into the native soil. In larger storm events, the
doff will flew directly into the wertmw inlet, whim is piped and directed to Springti ells public stormwater
main on Maple Island Road.
Component Definitions
• Rain Gardens are vegetated flat bottomed and shallow landscape depressions used to collect and
hold stormwater runoff.
• Growing Media is a specialty soil containing little to no fines designed to filter stormwater reliably
without clogging. H the planter soil needs to be replaced, It must be replaced with soil that matches
the building's plans and specifications. Growing medium to be 24" deep minimum.
• Overflow Inlet Avertical pipe with an atrium grate over It that allows stormwaterfmm large rain
events to bypass treatment and enter the downstream storm system. The grate prevents debris and
rodents from entering the piped storm system. The domed nature of the grate all smurages clogging

• Catch Basin is an l til at drain system that typically l ncludes a grate where stormwater enters the catch
basin, and a basin to capture sediment, debris, and associated pollutants. The purpose of the basin Is
to help prevent the downstream pipes from becoming clogged and to reduce the amount of seal ment
and debris being discharged Into our rivers and streams.

Inspection/Maintenance Schedule
The whole system shall be inspected and maintained annually and within 48 Fours after each major storm
event. For this 0&M plan, a major storm event Is helped as 2 Inches of rain In 24 hours or more. All
components of the storm system as described on the next page must be inspected and maintained
frequently, or they will cease to function effectively. The facility owner must keep a log recording all
inspection dates, observations and maintenance activities. Receipts shall be saved when maintenance is
performed sothere is a record of all expenses.
Procedure
Rain Garden
• See Appendix 1 torthe required maintenance procedure.
Inlets and Drains
• They should be l nspectedtor obstructions, debris, oil and grease, which should be removed upon
discovery.
• If water ponds overthe lnleHor more than two Fours aftera major storm, the Inlet drain may be
clogged.
• Checktor debris/sediment accumulation In and amundthe Inlet or overflow and rake and remove
upland causes (erosion, surface debris, etc).
Piped Storm System
• An annual Inspected mrdogging should be performed.
