You and your company have until January 2nd to revise and resubmit a complete application. Otherwise, you will put the
city in the awkward position to refer your business to our Code Enforcement Officers and to cease operations until the
application is complete, reviewed and approved, and all requirements instituted on-site.

As we discussed on the phone just now, I reviewed the DWP materials you sent via email and have some questions and
feedback for you. Currently the application is missing some elements and therefore is incomplete. My initial comments
are below. Please let me know if you have any questions.
o Application cover sheet: This form incorrectly identifies me ('Amy Chinitz') as the applicant rep. Please remove my
name and resubmit to Drew Larson with the correct information.
o Hozardous Moteriol lnventory Statement (HMIS): The application requires a complete HMIS with all hazardous
materials to be used/kept on site. The list in your narrative includes four chemicals (ethanol, acetone, heptane, and
toluene). The narrative also refers to isopropyl alcohol, which should also be included in the HMIS, along with any
other chemicals/haz mats to be used on site. I have attached an HMIS template provided by the fire department.
o Facility Plan:The map of the site should include all hazardous material storage and spill kit locations. The storage
locations identified in your facility plan should match those identified in your HMIS. ldeally, your facility plan should
also show storm drains so employees know where to go to protect a spill from entering the stormwater system.
When we spoke just now,lasked if the ethanolwas reallygoingto be stored outside as indicated in the narrative.
You responded that it will be stored in the building. ldentifying the location on the facility plan will clarify that
confusion.
o Borrowed Documents: Several of the documents you submitted come from the University of California Santa Barbara.
They referto phone numbers, personnel, procedures, and chemicals that do not apply to your facility (e,g., methyl
ethyl ketone, acids, and radioactive materials). lt is perfectly acceptable to borrow from other sources, but the
HMMP for your business needs to accurately represent the chemicals you have on site, your actual
procedures/policies, and your personnel and contact information. Again, I refer you to the HMMP template we have
discussed before. I have attached an example of how one business used that template (please note that it does not
include the page with the business information).
o As a reminder, here are the standards of approval for the 5-10 TOTZ. The last one, regarding inspection, requires you
to have an inspection check sheet. I did not see one in the documents you submitted. There is an example in the
HMMP template that you can adapt for your purposes:
o The storage, handling, treatment, use, production or otherwise keeping on premises of more than 20 gallons
of hazardous materials that pose a risk to groundwater in aggregate quantities not containing DNAPLS is
allowed upon compliance with containment and safety standards specified by the most recent Fire Code
adopted by the city.
o All hazardous or other materials that pose a risk to groundwater shall be stored in areas with approved
secondary containment in place (Springfield Fire Code 27O2.L and 27O4.2.2l,.
o All new use of DNAPLs are prohibited. Any change in type of use or an increase in the maximum daily
inventory quantity of any DNAPL is considered a new use and is prohibited'
oThe following requirements for inspection and record keeping procedures for monthly in-house inspection and
maintenance of containment and emergency equipment for all amounts of hazardous or other materials
that pose a risk to groundwater shall be met unless exempted: Schedules and procedures for inspecting
safety and monitoring and emergency equipment. The applicant shall develop and follow a written

inspection procedure acceptable to the Director for inspecting the facility for events or practices which
could lead to unauthorized discharges or hazardous materials. An inspection check sheet shall be developed
to be used in conjunction with routine inspections. The check sheet shall provide for the date, time, and
location of inspection; note problems and dates and times of corrective actions taken; and include the name
of the inspector and the countersignature of the designated safety manager for the facility.
I want to reiterate that it is important to submit documents that accurately represent your procedures and policies. Your
permit will have a statement like this:
The findings and conditions presented below are based on an evaluation of the submittal documents and the
expectation that all aspects of the hazardous materials management plan [HMMP) will be implemented as stated
and intended, unless amended by conditions listed below or by other conditions issued by the City of Springfield.
That means that if you submit plans that you don't actually intend to put into practice, you will not be compliant with
your DWP permit.
