Final Site Plan Review Application Process
1. Applicant Submits a Final Site Plan Review Application to the Development Services
Department
• The application must conform to the Final Site Plan Review Submittal Requirements
Checklist on page 4 of this application packet.
Planning Division staff screen the submittal at the front counter to determine
whether all required items listed in the Final Site Plan Review Submittal
Requirements Checklist have been submitted.
Applications missing required items will not be accepted for submittal.
2. City Staff Conduct Detailed Completeness Check
• Planning Division staff conducts a detailed completeness check within 30 days of
submittal.
• The assigned Planner notifies the applicant in writing regarding the completeness of
the application.
• An application is not be deemed technically complete until all information necessary
to evaluate the proposed development, its impacts, and its compliance with the
provisions of the Springfield Development Code and other applicable codes and
statutes have been provided.
• Incomplete applications, as well as insufficient or unclear data, will delay the
application review process and may result in denial.
3. City Staff Review the Application & Issues Final Approval
• This is a Type I decision and thus is made without public notice and without a public
hearing since there are clear and objective approval criteria and/or development
standards that do not require the use of discretion.
Decisions address all the applicable approval criteria and/or development standards.
Applications may be approved, approved with conditions, or denied.
The City mails the applicant and any party of standing a copy of the decision, which
is effective on the day it is mailed.
The decision issued is the final decision of the City and may not be appealed.
4. City Drafts a Development Agreement
Applicant and City sign Development Agreement
Final Site Plan and Development Agreement become null and void if construction
has not begun within two years of signing the Development Agreement.
5. City Conducts Final Site Inspection
• City authorizes provision of public facilities and services and issues Certificate of
Occupancy

CONDITION OF APPROVAL RESPONSE LETTER
The applicant, G Street Integrated Health, received approval of its Tentative Site Plan Review application
(Case No. 811-23015-TYP2) on March 8, 2023. It contained four (4) Conditions of Approval. Outlined
below, you will find the applicant's response to each of those Conditions.
1. The Applicant must submit a draft of the deed restriction tying the four lots together with the Final Site
Plan. After approval of the document the applicant must provide a copy of the recorded document to
the City prior to final occupancy.
The deed restriction is submitted with this Final Site Plan Review application for review and once staff
has cleared the restriction, the applicant will have it recorded and proof of recording will be returned
to the City.
2. A Standard Springfield Wellhead Protection Sign must be posted permanently at the trash enclosure
as applicable or other location to be determined in consuitation with SUB Drinking Water Protection
Coordinator. The applicant may contact SUB Water Department (541-726-2396) to obtain sign
standard orto purchase signs directly (at) SUB (fw) S1 &sign.
The applicant is aware of the need for the sign. Prior to building permit sign off the applicant will
contact SUB, purchase the sign, and post it on the proposed trash enclosure.
3. A Drinking Water Protection Application or Exemption must be submitted and approved.
This has been completed. Please see email from SUB representative, Amy Chinitz, dated March 22,
2023. The exemption has been granted.
4. On the Final Site Plan Submittal show all existing easements including the 7' wide perimeter PUE's
and the 10' wide PUE listed on the Marylhurst Plat.
The site plan has been updated and all PUE's have been included on the plans.

contiguous lots described in Statutory Warranty Deed recorded on August 31, 2022, Recorder's Reception
Number 2022-037556 (Map: 17-03-36-21, Tax Lots 04700, 04800, 05700, 05800) in Lane County Oregon
Deed Records, in Springfield, Lane County, Oregon.
FURTHER; the description of the subject properties are contained in said recorded document, Reception
No. 2022-037556 (Tax Lots 4700, 4800 and 5700, 5800) and are contiguous with each other and described
as:
Lots 1, 2, 11, and 12, Block 1, MARYLHURST, as platted and recorded in Book 12, Page 27, Lane County
Oregon Plat Records, in Lane County, Oregon.
NOW THEREFORE; In Consideration for the issuance of a Site Plan Review Approval by the City of
Springfield which allows the owner(s) to operate a building and parking lot across the above described real
property, the four (4) lots under the same ownership, the owner(s) do hearby make the following restriction
(for the good and safety of the public, in compliance with the Springfield Development Code and Building
regulations), specifying that these restrictions shall constitute a limitation which runs with the land and shall
be binding on all persons claiming under them, and the restrictions shall be for the benefit of present and
future owners of said real property.
BE IT HEREAFTER KNOWN and agreed that:
1. The land described above, which includes four (4) lots must either be sold together without
separation ofthe lots or portions thereof as a single unit of land so long as the building and required
parking exists on one site and the remainder of the associated uses exists on the other lots: or
2. Prior to the sale of the lots individually, the use shall be discontinued, and a finding made by the
City that there is no violation of any applicable building or parking code requirements applicable at
the time of sale.
