(TO BE UPDATED ANNUALLY)
The Willamette Valley Oregon Temple of The Church of Jesus Christ of Latter-day Saints is located in the
Sports Way/1-5 Wellhead Protection Area in the Gateway neighborhood of Springfield. The majority of
the site is in the 1 -year Time of Travel Zone, with the 2 -year Time of Travel Zone along the eastern
boundary. This means that chemicals that spill or leak onto the ground surface have the potential to
contaminate the well in some cases in less than a year's time. The church is a committed partner in the
effort to protect Springfield's drinking water supply. This hazardous materials management program
emphasizes the importance of preventing chemicals leaks and spills and responding promptly and
effectively should a release occur.
This Hazardous Materials
Management Plan (HMMP)
outlines the church's protocol for
preventing, controlling, collecting,
and disposing of any unauthorized
releases of hazardous materials.
The plan also identifies the
greatest areas of risk within the
facility and outlines the steps to
take to reduce the risk. Any
questions about this plan or how
the church manages its hazardous
materials should be directed to the
emergency contact individual on
the next page.

Part B — General Facility Description / Site Plan
The Church of Jesus Christ of Latter-day Saints new temple, the Wllamette Valley Oregon Temple, is
located on a 10.29 -acre site in the Gateway neighborhood of Springfield. The temple is approximately
30,000 square feet in size and serves some 30,000 Latter-day Saints. The attached site plan shows the
site and areas where hazardous materials may be stored.
Part C — Facility Map / Hazardous Material Storage Locations
The floor plans illustrate the location of all hazardous material storage areas, including hazardous waste
storage or holding areas, and spill kits. The storage areas identified on the facility map correspond with
those listed in the HMMP in Section II. Spill kits are placed strategically in custodial rooms, laundry
areas, basement areas and mechanical penthouse level so they can be accessed in areas where spills
are most likely to occur.

Section II: Hazardous Materials Inventory Statement (HMIS)
Products stored on site, as well as products used on site but not stored on site, are listed in the inventory
chart at the end of this document. Information includes:
• Manufacturer's Name / Product Name
• Hazardous Ingredients and Concentration
• Hazardous Classification Codes
• Chemical Abstract Services No.
• Maximum Quantity per Unit
• Approximate Range of Unit Sizes
• Storage Conditions
• State of Material
• Open or Closed Use
• NFPA 704 Classification
• Location of Chemical Stored
If hazardous material inventory changes, this HMIS shall be updated to reflect those changes and
provided to SUB for review for compliance with Springfield Development Code SDC 3.3-200 on an annual
basis.
Products containing DNAPLS are prohibited. Any new materials proposed for use or storage on-site
shall be evaluated for DNAPLS by the Springfield Utility Board Water Quality Protection Coordinator (541-
744-3745) prior to purchase.
No more than 500 gallons of hazardous materials can be kept on the premises. The maximum allowed
container size is 5 gallons. No more than 150 gallons of hazardous materials that pose a risk to
groundwater may be on the premises in opened containers for handling, treatment, use, production, or
dispensing on site.

Section V: Record Keeping (Inspection and Maintenance)
This section describes the procedures for monthly inspection and maintenance of containment and
emergency equipment and record-keeping procedures and include the inspection form(s) to be used.
Information addressed includes:
How frequently will inspections take place?
The visual inspections will take place monthly to ensure that chemicals aren't leaking and that
containment devices are intact. If any chemicals are found to be leaking or containment devices are
found to have lost their structural integrity, any leaks will immediately be cleaned up, and measures will
be taken to ensure that leaks have not penetrated the floor.
Who will conduct inspections? Who is responsible for signing off on completed inspection
forms?
The monthly inspections will be conducted by the Temple Facilities Senior Assistant, and the sign off will
be his/her boss, the Temple Facilities Manager (TFM).
Where will completed inspection forms be kept end for how long?
The completed inspection forms will be kept in the TFM's office and will be kept for three years plus the
current year.
What methods will be used to inspect secondary containment devices?
Because no bulk liquid chemicals (i.e., 55 -gallon drums) nor in -ground storage of chemicals will be used,
large secondary containment devices will not be necessary. Therefore, the trays and appropriate
cabinets (such as flammable chemical cabinets) which will be used can be effectively inspected visually.
Any leaks will be easily identifiable. Not only should a leak be looked for around the base of a container
(either original or secondary), but randomly, chemical and secondary containers should monthly be
picked up or moved to ensure that there is no leakage directly under the containers which is not visible
from just observing the outside of the container at the base. The secondary containment devices will also
be checked for cracks, discoloration, corrosion, etc. to be proactive so that if an original container were to
leak, the secondary containment device would be in proper condition to prevent any further leakage.
Since any storage of non -liquid chemicals does not require secondary containment devices, visual
inspection of the actual chemical containers will be sufficient and reliable.
What procedures will be followed for resolving detected problems?
If, during a monthly inspection or at any time, a leakage problem is observed, the original container must
immediately be removed and any chemical left in the container put into another properly labeled
appropriate container. The secondary container must immediately be cleaned out and inspected to
ensure that no chemical has escaped beyond the secondary container and that no damage has occurred
to the secondary container. Any absorbent materials and cleaning supplies used will be disposed of per
the above-mentioned protocol.

If the facility uses a sealed floor as secondary containment, this section should describe (or
reference an appendix that describes) the maintenance and repair protocol for the floor.
Although there will be sealed floors, they are not planned to be used as a secondary containment device.
Most floors will have something like a tile covering. The temple is planned as a one-story building (with a
very small basement at one end of the building) and underneath the slab there will be a vapor barrier.

Section VI: Spill Prevention
The spill prevention plan includes the following
The temple's spill prevention plan includes such measures as storing chemicals at eye level or below so
that any leaks or spills are easily noticed, whether during the monthly inspection or when accessing
chemicals. The storage shelves will be secured to the wall so that a bump orjolt to the shelving unit
(including earthquake) won't throw containers to the floor. Incompatible materials will be stored
separately from each other. As mentioned above, a monthly inspection will be performed in each area
where chemicals are stored to check original containers and secondary containment systems for leakage,
spills or deterioration so as to keep all containers in good condition.
Employees will be trained after hiring, and regularly after that, regarding handling and storage of
chemicals.
The temple property will contain three buildings, including the one-story 30,000 square foot temple (with a
very small basement), and two very small one-story buildings --a grounds building and a clothing shop. All
chemicals will be stored inside these buildings and none will be stored outside. A contractor will handle
all grounds keeping and no landscaping chemicals will be stored onsite.
A natural gas emergency generator will be housed in the equipment yard side of the Grounds Building.
All other appliances, such as a boiler and laundry equipment inside the temple will also be natural gas
operated, so there is no danger of leakage into the ground water. There will be no loading dock and very
minimal outdoor equipment, since all landscaping will be handled by an outside contractor.
There will be a few storm drains on the grounds and, of course, sanitary drains in the buildings. The
temple will not be using a lot of chemicals and will not have any bulk storage of chemicals, so the
possibility of a major release from a leak or accidental spill is very low. Nevertheless, absorbent material
for spills will be kept in the Grounds Building and everywhere inside the temple where chemicals will be
stored. Also, filter material will be stored in the Grounds Building for any major amount of debris that
might be headed for a storm drain to keep a majority of that debris from going down the drain.
