Dear Mr. Shiroyama:
This letteris to conf irm your telephone conversation with Dave Puent,Building 0fficial, on Friday, September 15, 1989.
It is our understanding that you intend to erect a satellite disheither on or near the K-Mart Build'ing located at 1401 North 21st
Street, Springfie'ld, Oregon. A building perm'it is not required to
erect a satellite dish, however, an electrical perm'it in the amount of
$15.75 w'ill need to be obta'ined prior to the connection of the dish.
The supervising electric'ian for the company which you choose to do the
connection must sign the permit application prior to issuance. I have
enclosed an appfication for your use.
Prior to the installat'ion of the dish, please contact either Greg
Mott or Cindie Harmon at 726-3759. They wi'|1 be able to inform you of
the required setbacks from your property lines if applicable.
