Permits shall be effective for one year from the date of issuance.
Upon completing the construction for which a permit has been issued the permit holder sha11 notify the
Department by subrnitting the installation record form. The Deparfmeqt sha11 inspect the construction to
Cetermine if it complies with the rules contatned,in this divisioft,; if tne construction does comply vrith
such rules, the Department shall issue a cert,ifr-cdte of satisfact#y completion to the permit holbei. If the.
construction does not cornply with such rules, the Department shall notify the permit hoider and shall requiresatisfactoiy coilplet'ion Ubtilre issiring the cirtificate. Fallure to meefthe rbquirements ror, iaiiifacto.v
completion within a reasonable t'ime constitutes a violation of ORS 454.605 lo 454.745 and this rule. 1
