Final Site Plan Review Application Process
1. Applicant Submits a Final Site Plan Review Application to the Development Services
Department
The application must conform to the Fina/ Site Plan Review Submittal Requirements
Checklist on page 4 of this application packet.
Planning Division staff screen the submittal at the front counter to determine
whether all required items listed in the Final Site Plan Review Submittal
Requirements Checklist have been submitted.
Applications missing required items will not be accepted for submittal
2. City Staff Conduct Detailed Completeness Check
• Planning Division staff conducts a detailed completeness check within 30 days of
submittal.
• The assigned Planner notifies the applicant in writing regarding the completeness of
the application.
• An application is not be deemed technically complete until all information necessary
to evaluate the proposed development, its impacts, and its compliance with the
provisions of the Springfield Development Code and other applicable codes and
statutes have been provided.
• Incomplete applications, as well as insufficient or unclear data, will delay the
application review process and may result in denial.
3. City Staff Review the Application & Issues Final Approval
• This is a Type I decision and thus is made without public notice and without a public
hearing since there are clear and objective approval criteria and/or development
standards that do not require the use of discretion.
• Decisions address all the applicable approval criteria and/or development standards.
• Applications may be approved, approved with conditions, or denied.
• The City mails the applicant and any party of standing a copy of the decision, which
is effective on the day it is mailed.
• The decision issued is the final decision of the City and may not be appealed.

CONDITIONS OF APPROVAL I RESPONSES
I. The Utility Plan, Sheet C3, has been updated to note the Encroachment Permit
requirement.
2. The information requested will be provided as part of the Encroachment Permit
application.
3. The Trash and Recycle enclosure is shown on the Site Plan, Sheet AO l.The enclosure is
detailed on Sheet A07.The sanitary drain is noted on Sheet C3.
4. The church will pursue alternatives to the discharge permit. Our civil engineer will
prepare the documentation to incorporate a weir overflow system.
5. The 20 -foot public drainage easement is noted on the revised Utility Plan, Sheet C3.
6. A draft O&M Agreement is attached for your review and will be finalized with the City.
7. A draft O&M Agreement is attached for your review and will be finalized with the City.
B. All required vegetation at the detention ponds will be fully vegetated and established
prior to final inspections and commencement of operations.
9. This is noted on the Details Sheet, C4.
10. We ask that this be deferred.This is part of the agreement with AKS and will be
completed prior to Final Occupancy of the building. We just received word from them
today that they are still "a month or so" out to complete this.
11. The PIP and complete rehabilitation of the Pierce Ditch channel and banks will be
completed prior to Final Occupancy of the building.
12. The Church will draft a Letter of Credit as assurance for the segment ofV Street along
the subject site frontage in accordance with the approved PIP plans by AKS Engineers.
This will be in place prior to issuance of Building Permits.
13. Refer to the PIP forV Street submitted by AKS Engineers separately for the water
system design serving the development site.
14. The 10 -foot public utility easement along Marcola Road is noted on the revised Utility
Plan, Sheet C3.
15. Both the 10 -foot and 7 -foot public utility easements will be recorded prior to Final
Occupancy of the building. Draft copies are enclosed with this application.
16. The Garage, Amphitheater, and Trash Enclosure plans and elevations are included on
Sheet A07.
17. See enclosed Site Lighting Plan, Sheet E01 and enclosed specifications.
18. The church is entering into a shared parking agreement for 40 additional spaces with
Volunteers in Medicine at the adjacent property across the Pierce Ditch to the west.This
will provide a total of 200 spaces on the main property, 39 spaces owned by the church
on the west side of the Pierce Ditch plus 40 additional shared spaces for a total of 279
available vehicle parking spaces, exceeding the 242 required.
19. A Drinking Water Exemption Permit will be pursued and approved prior to issuance of
building permits. If needed, in lieu of the exemption, a Drinking Water Protection Permit
will be obtained. Draft copies of the applications are enclosed with this application.
20. The required Vision Clearance Triangles are noted on the Site Plan Sheet A01 and the
Landscape Ran, Sheet L100.
21. The Pedestrian Bridge details are shown on Sheet A07.
22. The Pedestrian Bridge is designed to span the Ditch and be above the ordinary high
water mark to avoid awetland permit.

JOINT -USE PARKING AGREEMENT
PARTIES: NORTHWOOD CHRISTIAN CHURCH (Parcel A)

VOLUNTEERS IN MEDICINE (PEACEHEALTH) (Parcel B)

WHEREAS, it has been demonstrated to the Development Services Director (Director) that the
minimum parking requirements set forth in Section 4.6125 of the Springfield Development Code for
Parcel A, can be provided by Parcel B.
WHEREAS, Section 4.6-110 (E) the Springfield Development Code allows the Director, upon application
by all of the involved property owners, to authorizejoint use of the parking facilities, provided that:
1) This parking lot agreement shall be applicable only to forty spaces on Parcel B property as
designated by Volunteers in Medicine (Peacehealth).
2) The applicant shall demonstrate that there is no substantial conflict in the principal operating
hours of the facilities or uses for which the joint use of parking facilities is proposed; and
3) The parties concerned in thejoint use of on-site parking facilities shall provide evidence of
agreement for thejoint use by a legal instrument approved by the City Attorney. An agreement for
joint use of parking facilities shall provide for continuing maintenance of jointly used parking
facilities.
4) The agreement shall be recorded at Lane County Deeds and Records at the applicant's expense.
WHEREAS, the Director has allowed an exception to Section 4.6-125 of the Springfield Development
Code conditioned upon signing of this join -use parking agreement. This agreement authorizes parking
spaces on Parcel B to accommodate parking forthe uses by Parcel A, and the owner of Parcel B to
consent to allow said parking spaces upon the parcel to be used in the calculations of the requirements
for the uses by Parcel A and not to be included in the calculation of any uses upon Parcel B.
The Joint -Use Parking Agreement Shall become effective on January 1, 2023, this agreement insures to
the benefit of and binds the parties, their successors and assigns for so long as Parcel A uses require.
This Agreement may not be amended orterminated without approval of the Development Services
Director for the City of Springfield.

DWP Application Exemption Request Process
Commercial and industrial facilities located within the DWP Overlay District that do not store,
use, or produce more than twenty gallons of hazardous materials and that do not contain dense
non -aqueous phase liquids (DNAPLs) may choose to seek exemption from submitting a DWP
application. However, even if the applicant's exemption from submitting a DWP application is
approved, the applicant may still be subject to DWP regulations if the Development & Public
Works Director, in consultation with the Springfield Utility Board (SUB) and the Fire and Life
Safety Department, determine that an exempt hazardous material, activity, or facility has the
potential to degrade groundwater quality. In such instances, the DWP regulations will be
enforced concurrently with any associated land use application.
1. Applicant Submits a DWP Application Exemption Request to SUB
• The request must conform to the DWPApplication Exemption Request Submittal
Requirements Checklist on page 3 of this application packet.
• SUB conducts a detailed completeness check of the application.
• SUB notifies the applicant in the event that submittal requirements are missing or
unclear. Note: Sites may be inspected at any time to verify information provided in
the submittal, and applicants may be requested to submit some or all Material
Safety Data Sheets (MSDS) for items listed on the list of hazardous materials.
• An application is not deemed technically complete until all information necessary to
evaluate the request has been provided to SUB.
• Incomplete applications, as well as insufficient or unclear data, will delay the
application review process and may result in denial.
3. SUB Reviews the Application and SUB and the City Issue a Decision
SUB notifies the applicant and the Planning Division in writing regarding the decision
to grant the exemption or not.
The Planner assigned to the associated land use case incorporates the exemption
decision and any applicable findings and conditions into the land use decision.
DWP application exemptions are not granted in perpetuity. If the conditions of
exemption are deemed to have not been met at any point in time by SUB, a DWP
application will be required.

2. City Staff Conduct Detailed Completeness Check
• Planning Division staff conducts a detailed completeness check within 30 days of
submittal.
• The assigned Planner notifies the applicant in writing regarding the completeness of
the application.
• An application is not be deemed technically complete until all information necessary
to evaluate the proposed development, its impacts, and its compliance with the
provisions of the Springfield Development Code and other applicable codes and
statutes have been provided.
• Incomplete applications, as well as insufficient or unclear data, will delay the
application review process and may result in denial.
3. City Staff Review the Application and Issue a Decision
• This is a Type I decision and thus is made without public notice and without a public
hearing since there are clear and objective approval criteria and/or development
standards that do not require the use of discretion.
• Decisions address all the applicable approval criteria and/or development standards.
• Applications may be approved, approved with conditions, or denied.
• The City mails the applicant and any party of standing a copy of the decision, which
is effective on the day it is mailed.
• The decision issued is the final decision of the City but may be appealed within 15
calendar days to the Hearings Official.

NOTE: If you feel an item does not apply, please state the reason why and attach the
explanation to this form.
❑ Submitted Concurrently with Site Plan Review or Minimum Development Standards
applications, where applicable.
❑ Application Fee - refer to the Development Code Fee Schedule for the appropriate fee
calculation formula. A copy of the fee schedule is available at the Development & Public
Works Department. The applicable application, technology, and postage fees are collected
at the time of complete application submittal.
❑ DWP Overlay District Development Application Form
❑ Copy of the Deed
❑ Copy of a Preliminary Title Report issued within the past 30 days documenting
ownership and listing all encumbrances.
❑ Narrative explaining the proposal and any additional information that may have a bearing
in determining the action to be taken, including findings demonstrating compliance with
the Standards for Hazardous Materials within Time of Travel Zones (TOTZ) listed in SDC
3.3-235.
NOTE: Before the Director can approve a DWP Overlay District Development request,
information submitted by the applicant must adequately support the request. All of the
Standards for Hazardous Materials within TOTZ must be addressed. Incomplete
applications, as well as insufficient or unclear data, will delay the application review
process and may result in denial. In certain circumstances, it is advisable to hire a
professional planner or land use attorney to prepare the required findings.
❑ Three (3) Copies of a Plot Plan reduced to 8 1/2' by 11" outlining the applicable TOTZ
boundaries in relationship to the property boundaries
❑ Three (3) Copies of the Following:
❑ Use of hazardous materials that pose a risk to groundwater that existed on the
property prior to May 15, 2000, if applicable
❑ Hazardous Material Inventory Statement and Material Safety Data Sheet for all
materials entered on the Statement. Hazardous material weights shall be converted to
volume measurement for purposes of determining amounts (i.e. 10 pounds shall be
considered equal to one gallon)
❑ List of chemicals to be monitored through the analysis of groundwater samples and a
monitoring schedule if groundwater monitoring is anticipated to be required
❑ Detailed description of the activities conducted at the facility that involve the storage,
handling, treatment, use, or production of hazardous materials in aggregate quantities
greater than 500 gallons
❑ Description of the primary and any secondary containment devices proposed, and, if
applicable, clearly identified as to whether the devices will drain to the sanitary sewer
or stormwater system

❑ Proposed Hazardous Material Management Plan for the facility that indicates
procedures to be followed to prevent, control, collect, and dispose of any unauthorized
release of a hazardous material
❑ Description of the procedures for inspection and maintenance of containment devices
and emergency equipment
❑ Description of the plan for disposition of unused hazardous materials or hazardous
material waste products over the maximum allowable amounts, including the type of
transportation and proposed routes
